The earliest recorded observations resembling amblyopia appear in ancient Greek medical texts. The word itself derives from the Greek 'amblys,' meaning dull, and 'ops,' meaning eye or vision, a terminology that reflected the puzzling clinical reality physicians encountered: an eye that looked normal yet saw poorly. Hippocratic writers noted cases of reduced vision without visible structural explanation, and later ancient physicians grappled with how to account for such findings within prevailing theories of optics and vision.

Galen of Pergamon, writing in the second century CE, provided the most influential ancient account of visual physiology, proposing that vision depended on a 'visual spirit' or pneuma transmitted from the brain through the optic nerves to the eyes. Within this framework, amblyopia could be understood as a weakness or deficit in the transmission of visual spirit, a conceptualization that, while incorrect by modern standards, offered physicians a theoretical basis for the condition that persisted through much of the medieval period.

Medieval Islamic scholars, particularly Ibn Sina and Ibn al-Haytham, made significant contributions to the understanding of vision and its disorders. Ibn al-Haytham's Book of Optics, composed in the early eleventh century, represented a remarkable advance in the physics of light and vision, emphasizing that vision arose from light entering the eye rather than rays emanating from it. While Ibn al-Haytham's work did not specifically address amblyopia as a clinical entity, his systematic approach to visual science laid groundwork that later European scholars would build upon.

During the Renaissance and early modern period, growing anatomical knowledge of the eye slowly enriched discussions of visual impairment. However, amblyopia remained poorly understood precisely because dissection revealed no obvious structural lesion to explain it. Physicians and natural philosophers debated whether the condition originated in the eye itself, in the optic nerve, or in some quality of the visual spirit.

The seventeenth and eighteenth centuries brought increasingly sophisticated optical theory and the first systematic clinical descriptions of amblyopia in children with strabismus. Physicians began observing that children with a turned eye often appeared to suppress vision in that eye, and that the suppression seemed related to the brain's apparent preference for the image from the straighter eye. The French physician Georges-Louis Leclerc, Comte de Buffon, discussed the relationship between strabismus and visual suppression in the eighteenth century, contributing to emerging clinical awareness.

The French ophthalmologist Louis Émile Javal and others in the nineteenth century conducted detailed studies of strabismic patients and noted the association between misalignment and reduced visual acuity in the deviating eye. The concept of occlusion therapy, whereby the stronger eye was covered to force use of the weaker one, had roots stretching back to at least the eighteenth century, with Buffon and the English surgeon John Hunter both having discussed or employed variants of patching. The theoretical understanding of why occlusion might work deepened through the nineteenth century as neurophysiology advanced.

By the late nineteenth and early twentieth centuries, ophthalmologists had accumulated substantial clinical experience with amblyopia, and the condition was increasingly framed in terms of cortical suppression and visual pathway development rather than humoral deficits or optical physics alone.
