In the early twentieth century, interest in the condition grew as neuropathological techniques improved and as clinicians in Europe and North America accumulated larger case series. The diagnosis of Lou Gehrig in 1939 brought the disease to public consciousness in the United States in a way that purely academic publications never had, and charitable and research organizations began forming in subsequent decades partly as a result of that cultural moment. Researchers in the mid-twentieth century began examining whether geographic clusters of the disease, particularly the unusually high prevalence observed among the Chamorro people of Guam during the 1940s and 1950s, might offer clues to underlying causes. Studies by Leonard Kurland and Donald Mulder at the National Institutes of Health investigated this Guamanian cluster extensively, and their work prompted hypotheses about environmental and nutritional factors, including the possible role of a neurotoxic amino acid found in cycad seeds consumed in the local diet. The discovery in 1993 by a research team that included Robert Brown that mutations in the gene encoding the enzyme superoxide dismutase 1 were associated with familial forms of the disease represented a landmark molecular turning point, shifting scientific understanding toward genetic and biochemical frameworks that had been largely absent from earlier centuries of inquiry. Across roughly 150 years of organized investigation, the condition moved from a clinically described curiosity in a Parisian hospital to a subject of international genetic and neurochemical research.
