Egyptian healers believed the heart governed the body's vital fluids, and pallor was understood as a failure of the heart's nourishing function rather than any deficiency in the blood itself.
Ancient Greek physicians, working within the framework of humoral medicine established by Hippocrates and later elaborated by Galen, interpreted pale, weakened patients as suffering from an excess of phlegm or a deficiency of blood among the four humors. Galen, writing in the second century CE, prescribed diet, exercise, and bloodletting in certain cases, believing that stimulating the body could restore humoral equilibrium. The Renaissance brought renewed anatomical interest but little immediate clarification. William Harvey's 1628 demonstration of blood circulation transformed understanding of how blood moved through the body but did not yet illuminate what blood was made of or why it sometimes failed in its duties.
The invention of the microscope opened an entirely new chapter. Antonie van Leeuwenhoek in the 1670s became the first to observe red blood corpuscles, describing their disc-like shape and estimating their enormous numbers. This observation planted the seed for a cellular understanding of blood, though decades passed before anyone connected deficiencies in these corpuscles to disease states.
In the early nineteenth century, French physician Gabriel Andral conducted systematic microscopic examinations of blood from patients suffering from various wasting conditions and documented reductions in red corpuscles, publishing his findings in the 1840s. Thomas Addison's 1855 description of a fatal form of anemia associated with degeneration of the gastric mucosa marked another watershed, isolating what later became known as pernicious anemia as a distinct and devastating entity. George Whipple, George Minot, and William Murphy would eventually unravel the role of dietary liver in treating pernicious anemia in the 1920s, work for which they shared the Nobel Prize in Physiology or Medicine in 1934 — a recognition that signaled anemia's full emergence as a scientifically understood family of conditions rather than a mysterious humoral imbalance.
