The earliest references to what may have been angiosarcoma appear scattered across ancient medical literature, though positive identification is complicated by the non-specific language ancient healers used to describe malignant growths. Egyptian papyri, including the Edwin Smith Papyrus dating to approximately 1600 BCE, described ulcerating tumors that could not be treated, and while the authors made no mechanistic distinctions between tumor types, some lesions they described were later reinterpreted by twentieth century medical historians as potentially vascular in origin. Greek physician Hippocrates established the word cancer, derived from the Greek word for crab, to describe hard, spreading tumors, and his followers applied this terminology broadly to any mass that seemed to invade surrounding tissues, making it impossible to retrospectively identify angiosarcoma as a distinct entity within the Hippocratic corpus.

Galen of Pergamon, writing in the second century CE, elaborated on Hippocratic tumor theory by attributing cancers to an excess of black bile, the coldest and driest of the four humors. This framework governed medical thinking about malignancy across Europe and the Islamic world for well over a millennium, and physicians working within it had neither the incentive nor the methodology to subdivide tumors based on their cellular origin. Medieval Arabic physicians including Al-Zahrawi described surgical approaches to surface tumors and ulcerations, and some of the aggressive, rapidly ulcerating growths he documented may have included angiosarcomas of the skin, though his descriptions remained anchored to observable surface features rather than internal vascular architecture.

The invention of the compound microscope in the seventeenth century and its gradual application to medical investigation in the eighteenth and nineteenth centuries created the conditions necessary for a new era of tumor classification. Giovanni Battista Morgagni, the eighteenth century Italian anatomist often called the father of pathological anatomy, emphasized the importance of correlating clinical observations with post-mortem tissue findings, laying groundwork that future pathologists would build upon to distinguish between tumor types. Rudolf Virchow, the towering nineteenth century German pathologist who established the cellular theory of disease, further advanced this project by insisting that all disease arose from changes in individual cells, and his framework made it conceptually possible to classify tumors by the specific cell type from which they originated.

It was within this Virchowian tradition that late nineteenth century pathologists began distinguishing endothelial cell tumors from other sarcomas, gradually accumulating case reports of highly vascular malignancies that seemed to arise from the vessel walls themselves. The term angiosarcoma came into use in the early twentieth century as histological staining techniques improved enough to allow pathologists to confirm the endothelial origin of these cells. Subsequent decades brought increased recognition of particular exposure histories associated with the condition, most notably the observation reported in the 1970s linking hepatic angiosarcoma to occupational exposure to vinyl chloride in manufacturing workers, a finding that connected industrial toxicology to this rare malignancy and reshaped research directions for the remainder of the century.
