The ancient world possessed no concept of aphasia as a neurological phenomenon distinct from madness, dementia, or the general dissolution of mental faculties. Egyptian medical papyri, including the Edwin Smith Papyrus dating to approximately 1700 BCE, described loss of speech following head trauma, making it among the earliest recorded observations of acquired language impairment attributable to brain injury. Greek physicians including Hippocrates noted that injuries to one side of the head could produce contralateral bodily consequences, and occasional references to loss of speech appeared in the Hippocratic corpus, though the mechanism was attributed to humoral disruption rather than localized brain damage.

For much of the medieval and early modern periods, loss of speech was categorized among general disorders of the rational soul or the animal spirits coursing through cerebral ventricles. The eighteenth century brought renewed anatomical interest in the brain's solid structures, and Franz Joseph Gall's development of phrenology in the late eighteenth and early nineteenth centuries, though ultimately discredited, had the historically significant effect of directing serious scientific attention toward the idea that specific mental functions were localized to specific brain regions. Gall reportedly identified a patient who had lost the ability to remember proper names following brain injury, a clinical observation that anticipated the specific features of anomic aphasia, and he attributed verbal memory to the frontal regions of the brain.

The pivotal transformation in understanding language disorders came in 1861 when the French surgeon and anthropologist Paul Broca presented his case of a patient known as Tan, who had been almost entirely unable to produce speech. Broca's postmortem examination revealed a lesion in the posterior inferior frontal gyrus of the left hemisphere, establishing for the first time through rigorous clinical and pathological correlation that a specific language function was localized to a specific brain region. This finding opened a era of systematic aphasia research. Carl Wernicke in Germany published his landmark work in 1874 describing a different form of aphasia associated with posterior temporal lesions, and he also began constructing theoretical models connecting different aphasic syndromes to distinct anatomical disruptions.

The concept of anomic aphasia as a specific syndrome — relatively preserved fluency alongside disproportionate difficulty retrieving words — was progressively carved out of the broader aphasic spectrum through the late nineteenth and early twentieth centuries. The British neurologist Henry Head criticized earlier classificatory schemes as overly rigid and proposed revised frameworks based on extensive clinical observation of brain-injured soldiers following the First World War. Head's work, published in his 1926 monograph Aphasia and Kindred Disorders of Speech, helped shift attention toward the functional dimensions of word retrieval failure. Norman Geschwind's influential reformulations of aphasiology in the 1960s and 1970s ultimately provided the connectionist framework within which anomic aphasia was formally situated as a distinct clinical entity within the modern classification of aphasic syndromes.
