Historical Narrative

The earliest documented encounters with what later generations would recognize as aortic aneurysm appear in ancient Egyptian medical texts, where healers described pulsating swellings of the torso that resisted all known remedies. These observations, recorded on papyrus scrolls dating to approximately 1550 BCE, reflected a world in which the heart and its great vessels were understood through spiritual as much as physical frameworks. Egyptian physicians attributed such swellings to imbalances of vital essences or the influence of malevolent forces, and treatments consisted primarily of herbal poultices and ritual incantations.

Ancient Greek physicians brought a more systematic, if still incomplete, understanding to vascular disease. Hippocrates and his followers catalogued pulsating masses in their clinical records, and Galen of Pergamon, working in the second century CE, described what he believed to be dilations of arterial vessels in several of his anatomical treatises. Galen's interpretation was rooted in his humoral framework, which attributed such abnormalities to an excess of blood or a weakness in the vessel walls caused by corrupt humors. His authority was so commanding that these explanations persisted largely unchallenged through the medieval period in both European and Islamic medical traditions.

Islamic physicians of the medieval golden age, including Ibn Sina, whose Canon of Medicine was completed around 1025 CE, preserved and elaborated on Galenic descriptions of arterial swellings. Ibn Sina attempted to classify pulsating masses by their location and behavior, though without the benefit of systematic dissection, his categories blended conditions that later anatomy would distinguish sharply.

The Renaissance transformation of anatomy, driven most decisively by Andreas Vesalius in his landmark 1543 work De Humani Corporis Fabrica, gave European physicians their first rigorously accurate maps of the aorta and its branches. Vesalius corrected numerous Galenic errors and provided detailed illustrations of the great vessels, laying groundwork that made more precise identification of aortic pathology theoretically possible. Subsequent anatomists, including Ambroise Paré in the sixteenth century, documented cases of arterial aneurysms encountered both in living patients and in post-mortem dissections, noting their tendency toward fatal rupture.

The seventeenth and eighteenth centuries brought increasing surgical ambition. Antonio Valsalva, the Italian anatomist whose name became attached to a maneuver still studied in medical history, contributed descriptions of aortic pathology in the early eighteenth century. His pupil Giovanni Battista Morgagni systematically correlated clinical observations with autopsy findings, publishing in 1761 his foundational De Sedibus et Causis Morborum, which included detailed accounts of aneurysmal disease and firmly established the principle of anatomical localization in pathology.

By the late eighteenth century, surgeons such as John Hunter in Britain had begun experimenting with proximal arterial ligation as a means of treating peripheral aneurysms, reasoning that reducing blood pressure in the affected vessel might promote clotting and stabilization. Hunter's 1785 operation on a popliteal aneurysm became a landmark in surgical history, and the principle was cautiously extended to other vessels in subsequent decades, though application to the aorta itself remained beyond the technical and physiological reach of nineteenth-century medicine. The true surgical era for the aorta would wait until the twentieth century.
