For most of recorded medical history, children who experienced progressive loss of vision, intellectual decline, and seizures were grouped under general labels such as idiocy, amaurotic family idiocy, or simply the effects of divine punishment or moral failing in their parents, depending on the cultural framework of the time. Ancient and medieval physicians had no means of distinguishing between the many conditions that produced childhood neurological deterioration, and records from Hippocratic writings through medieval European medical compendia described such children only in the broadest terms, attributing their deterioration to corrupted vital spirits, malformed humors, or hereditary taint.

The late nineteenth century brought important attempts to classify degenerative neurological conditions more rigorously. German physician Bernard Sachs and Polish physician Warren Tay independently contributed to characterizing what became known as Tay-Sachs disease in the 1880s and 1890s, and this work sensitized the European and American medical communities to the idea that distinct hereditary diseases of the nervous system existed within what had previously been a undifferentiated mass of childhood neurological decline.

In 1903, British neurologist Frederick Eustace Batten published a detailed account of several children who presented with progressive visual failure and neurological deterioration but whose pathological findings differed from those seen in Tay-Sachs disease. Batten observed characteristic changes in the nervous tissue and emphasized that this represented a separate clinical and pathological entity. His publication appeared in the Transactions of the Ophthalmological Society of the United Kingdom and drew attention to the retinal and cerebral features that distinguished these cases.

Around the same period, German physician Heinrich Vogt contributed additional clinical and hereditary analyses, and the condition became known for a time as Batten-Vogt disease or Spielmeyer-Vogt disease after Walter Spielmeyer, another German neuropathologist who provided further pathological detail in the early twentieth century. Spielmeyer's meticulous histological work in the 1900s and 1910s helped establish that the nervous tissue changes involved lipid-like storage material accumulating within cells, though the precise biochemical nature of that material remained unknown for many subsequent decades.

Throughout the mid-twentieth century, neuropathologists refined the classification of what were called neuronal ceroid lipofuscinoses — a group of storage disorders characterized by the accumulation of autofluorescent material in neurons and other cells. Researchers including Jan Zeman and Peter Dyken contributed taxonomic work in the 1960s that helped systematize the various clinical forms, including those affecting infants, children, and juveniles, recognizing that these represented related but distinct conditions sharing a common pathological signature.

Electron microscopy, which became widely available to research laboratories in the 1950s and 1960s, allowed investigators to visualize the distinctive ultrastructural inclusions within affected cells with far greater clarity than light microscopy had permitted. The curvilinear, fingerprint, and rectilinear profiles observed under electron microscopy became important research tools for distinguishing among the subtypes and connecting observations across different patient populations in Europe and North America.

By the end of the twentieth century, molecular genetic techniques had begun to locate the chromosomal regions associated with the various forms, transforming Batten disease from a purely clinical and histological description into a condition understood through the lens of specific genetic loci, though that molecular work extended well beyond the historical window that shaped the disease's foundational identity.
