Among the earliest written references to a condition resembling beriberi appeared in Chinese medical texts dating back several thousand years, where physicians described weakness and swelling in the limbs alongside profound fatigue. The Nei Ching, the ancient Chinese medical canon, contained passages interpreted by later scholars as describing a syndrome consistent with the disease. In these early frameworks, such ailments were understood through humoral or elemental theories, and healers attributed the suffering to imbalances of vital forces rather than to any deficiency in food.

As the centuries progressed, beriberi became a persistent scourge among populations across South and Southeast Asia, afflicting sailors, soldiers, prisoners, and the urban poor who subsisted heavily on white polished rice. Dutch colonial physicians working in the East Indies during the nineteenth century encountered the disease constantly among their patients and remained deeply confused about its origins. The prevailing germ theory of disease, which had achieved tremendous success in explaining cholera and tuberculosis, led many researchers to search vigorously for a bacterial or parasitic cause of beriberi, a pursuit that consumed considerable scientific effort and led nowhere productive.

The pivotal breakthrough came through the work of Christiaan Eijkman, a Dutch physician stationed in Java in the 1880s and 1890s. While working at a prison hospital, Eijkman observed that chickens fed on polished rice developed a paralytic condition resembling beriberi in humans, and that these same chickens recovered when given unpolished rice. He initially interpreted this finding through the lens of his time, theorizing that polished rice contained a toxin that was neutralized by something present in the rice bran. His colleague Gerrit Grijns later refined this interpretation significantly, proposing in 1901 that beriberi resulted not from a poison but from the absence of an essential protective substance found in rice bran and other foods. This represented a conceptual revolution, shifting the cause from something harmful that was present to something beneficial that was absent.

In the early twentieth century, the Polish-American biochemist Casimir Funk isolated what he believed to be the curative substance from rice bran and coined the term "vitamine" in 1912, proposing that beriberi and several other diseases were caused by the lack of such vital amines in the diet. This theoretical framework gave birth to the broader field of vitamin research. The specific factor responsible for preventing beriberi was eventually isolated and chemically characterized during the 1920s and 1930s by researchers including Barend Jansen and Willem Donath, who isolated the substance from rice bran, and later by Robert Runnels Williams, who determined its chemical structure and synthesized it in 1936, naming it thiamine.

The history of beriberi's conquest also unfolded dramatically in military contexts. The Japanese naval physician Kanehiro Takaki dramatically reduced beriberi among Japanese sailors in the 1880s by altering the naval diet, though he incorrectly attributed his success to increased protein rather than the incidental addition of thiamine-containing foods. His practical success nonetheless preceded the theoretical explanation by decades and saved thousands of lives, illustrating how empirical observation in medicine could outpace scientific understanding by a considerable margin.
