Historical Narrative

Ancient evidence for brucellosis extends further back than the modern era of bacteriology by several millennia. Paleopathological studies conducted in the late twentieth and early twenty-first centuries identified skeletal lesions in human remains from archaeological sites in the ancient Near East and Mediterranean that researchers interpreted as consistent with chronic brucella infection. Ancient Roman and Greek writers described febrile illnesses of an intermittent character common among populations living closely with goats and cattle, though these descriptions lacked the specificity later generations would bring to the condition.

The disease entered the formal medical literature in a sustained way through the experience of British military forces stationed on the island of Malta during the nineteenth century. Soldiers and sailors garrisoned there suffered in large numbers from a debilitating fever that recurred in waves, and the condition became known as Malta fever or Mediterranean fever. The garrison's medical officers accumulated extensive case records, recognizing that the illness was particularly prevalent in the island environment and that it posed a serious threat to military readiness.

David Bruce, a British Army surgeon, conducted the investigations on Malta that would ultimately bear his name. In 1887, Bruce isolated a small coccobacillus from the spleens of soldiers who had died from Malta fever and named it Micrococcus melitensis, after the Latin name for Malta. His identification of the causative organism represented a landmark in the bacteriological investigation of the disease, though the chain of transmission from animals to humans remained obscure for nearly another two decades.

The connection between infected goats and human illness was established by Themistocles Zammit, a Maltese physician and archaeologist working with a British Royal Commission in the early 1900s. Zammit demonstrated in 1905 that Maltese goats harbored the organism and that raw goat milk served as the primary vehicle by which the bacterium historically passed into human populations on the island. This discovery transformed understanding of the disease and led British authorities to prohibit the use of raw goat milk among military personnel on Malta, with dramatic consequences for case rates among the garrison.

Parallel investigations in the United States and Europe during the same era revealed related organisms affecting cattle and swine. Bernhard Bang, a Danish veterinarian, had described a brucella-like bacterium responsible for epidemic abortion in cattle herds, a condition that became known as Bang's disease. American researchers eventually demonstrated that the bovine and porcine organisms were closely related to Bruce's original isolate, and the genus Brucella was established to unify these discoveries under a single taxonomic framework honoring Bruce's foundational work.

Through the early and middle twentieth century, brucellosis remained a significant occupational concern in the historical literature, appearing frequently in accounts of slaughterhouse workers, veterinarians, and dairy farmers who encountered infected animals. Public health campaigns in multiple countries focused heavily on pasteurization programs and veterinary eradication efforts, with historical records documenting the gradual decline of the disease in cattle herds across North America and Western Europe as these programs took hold. The organism's historical ability to persist in animal reservoirs meant that eradication remained an ongoing project rather than a completed achievement through much of the century.
