The transition from fatalistic acceptance of cancer as an untreatable affliction to a condition subject to systematic scientific investigation unfolded over many centuries of gradual accumulation of anatomical knowledge, surgical technique, and eventually laboratory science.

Historical Narrative

The earliest known written descriptions of cancer-like conditions appeared in ancient Egyptian medical papyri. The Edwin Smith Papyrus, dated to approximately 1600 BCE but believed to be a copy of texts from as early as 3000 BCE, described bulging tumors of the breast that Egyptian physicians recognized as having no satisfactory treatment. The Ebers Papyrus, dating to roughly the same period, recorded treatments for various growths and ulcerations using topical applications of substances including arsenic paste, a remedy that persisted in various forms through Western medical practice for millennia. Egyptian physicians distinguished between treatable surface ulcers and deeper, harder masses they considered hopeless, suggesting an early empirical appreciation of malignant behavior.

Greek physicians built upon Egyptian and Near Eastern foundations, embedding cancer within the theoretical framework of humoral medicine. Hippocrates, in writings from the fifth and fourth centuries BCE, used the term karkinos — the Greek word for crab — to describe hard, spreading tumors, allegedly because their projecting vessels resembled crab legs. He attributed such growths to an excess of black bile, one of the four fundamental bodily humors. This explanation proved extraordinarily durable: Galen of Pergamon, writing in the second century CE, elaborated extensively on the humoral theory of cancer, arguing that localized accumulations of corrupted black bile gave rise to malignant tumors. Galen recommended dietary and purgative treatments aimed at eliminating the excess black bile, along with surgical removal where feasible. His authority was so great that Galenic humoral explanations for cancer remained dominant in European medical thought for over a thousand years.

Arabic physicians of the medieval Islamic world preserved, translated, and extended Greek medical knowledge during the early medieval period. Avicenna, writing in the eleventh century, described various tumor types in his Canon of Medicine and recommended surgical excision combined with cauterization as the primary intervention for accessible growths. European medieval physicians largely worked within the same humoral framework, and surgery for tumors, while practiced, was viewed with considerable pessimism.

The Renaissance brought renewed interest in direct anatomical observation. Andreas Vesalius, whose landmark work De Humani Corporis Fabrica appeared in 1543, corrected numerous Galenic anatomical errors through cadaveric dissection, though this did not immediately overturn humoral cancer theory. The development of the microscope in the seventeenth century eventually made possible a fundamentally different approach to understanding tissue abnormalities.

Johann Peter Frank and other eighteenth-century physicians began investigating environmental and occupational associations with cancer. Percivall Pott's 1775 observation that chimney sweeps in London developed scrotal cancer at unusually high rates represented a landmark moment: it was among the first systematic proposals that specific external exposures could cause cancer, predating modern carcinogenesis concepts by nearly two centuries.

The nineteenth century transformed cancer research. Johannes Müller's microscopic work in the 1830s established that cancers were composed of cells, and Rudolf Virchow's cellular pathology framework proposed that cancerous cells arose from pre-existing cells rather than spontaneous generation. The development of anesthesia in the 1840s and antiseptic surgical techniques by Joseph Lister in the 1860s made more extensive cancer surgery possible. Wilhelm Conrad Röntgen's discovery of X-rays in 1895 opened an entirely new investigative and therapeutic era, leading rapidly to the recognition that radiation could both cause and potentially treat certain malignancies.
