When he proposed that the disease was a significant cause of goiter in Brazil, this claim was challenged and ultimately disproven, and political opponents used the dispute to mount attacks on his broader work. A 1923 public debate in Rio de Janeiro drew sharp criticism of Chagas's epidemiological claims, and though his core parasitological discoveries were never seriously questioned, his reputation suffered during his lifetime from these controversies.

The decades following Chagas's death in 1934 saw gradual clarification of the disease's natural history. Researchers including Emmanuel Dias, another product of the Oswaldo Cruz Institute, devoted careers to demonstrating that the chronic cardiac and digestive manifestations observed in rural Brazilian populations were sequelae of earlier infection. Dias also led pioneering vector control campaigns in the 1940s and 1950s, using insecticides to reduce triatomine populations in domestic settings, establishing that interrupting transmission was feasible on a programmatic scale.

The understanding of the disease's cardiac pathology deepened through the mid-twentieth century as Brazilian cardiologists including Fritz Köberle conducted extensive pathological studies demonstrating that T. cruzi infection caused destruction of the autonomic nervous system of the heart and gastrointestinal tract, offering an explanation for the enlarged hearts and dilated esophagi and colons that characterized chronic cases. Köberle's neurogenic theory, proposed in the 1950s and 1960s, became highly influential even as later researchers continued to debate the relative contributions of neuronal destruction, direct myocardial damage, and autoimmune processes to the chronic disease picture.

International recognition of the disease's geographic scope expanded slowly through the latter twentieth century as immigration brought cases to regions where no vector transmission occurred, prompting historians and retrospective analysts to speculate about how long the disease had shaped the demography and health of Latin American populations.
