Historical Narrative

The earliest surviving description of what historians believe to have been coeliac disease was recorded by the Greek physician Aretaeus of Cappadocia in the second century CE. Writing in his treatise on chronic diseases, Aretaeus described a mysterious ailment he called the 'koiliakos' — derived from the Greek word for abdomen — characterised by a failure of the gut to retain nourishment and a progressive wasting of the body. He observed that the condition appeared more frequently in women and children, and that sufferers passed pale, foul-smelling stools. His description remained one of the most referenced accounts for well over a thousand years, though no effective intervention followed from it.

Throughout the medieval period, conditions resembling this wasting illness were loosely categorised under humoral frameworks. Physicians attributed chronic digestive failure to an imbalance of cold and moist humours in the bowel, prescribing warming foods, spiced wines, and purgatives intended to restore balance. No distinction was drawn between what might today be understood as separate gastrointestinal conditions, and the specific relationship between food and the illness remained entirely unrecognised.

In 1888, the English physician Samuel Gee delivered a landmark clinical lecture at the Hospital for Sick Children in London in which he offered a precise and detailed description of the coeliac affection in children. Gee noted the characteristic features of the illness with remarkable clinical accuracy for the era and, crucially, speculated that the solution to the condition might lie in diet. He famously wrote that 'if the patient can be cured at all, it must be by means of diet.' Gee experimented with various dietary regimens in his young patients, including one child whom he believed improved substantially on a diet of Dutch mussels — an observation he could not ultimately explain or replicate consistently.

The early twentieth century saw further investigation by the American physician Christian Archibald Herter, who published a monograph in 1908 examining the condition in children. Herter proposed that bacterial overgrowth in the intestine played a causal role, a theory that proved incorrect but kept scientific attention focused on the gut. For a time, the illness was informally referred to as 'Gee-Herter disease' in honour of both men's contributions.

The most transformative breakthrough came during and after the Second World War, through the observations of Dutch paediatrician Willem-Karel Dicke. Working in the Netherlands during the wartime famine of 1944, Dicke noticed that children who had previously suffered from the coeliac condition showed marked clinical improvement when wheat and rye bread became unavailable due to food shortages, only to deteriorate again when Allied airdrops restored grain supplies. Dicke pursued this observation systematically and, collaborating with biochemist Johan van de Kamer and paediatrician Hendrik Weijers, published findings in the early 1950s demonstrating that wheat flour — and specifically its gluten fraction — was responsible for provoking the intestinal condition. This work, contained in Dicke's 1950 doctoral thesis, fundamentally redirected research and clinical practice and is regarded as the pivotal turning point in the condition's history.

Subsequent decades saw the development of intestinal biopsy techniques, pioneered by Margot Shiner and later refined by Paulley and others, which allowed physicians to observe the characteristic flattening of the intestinal lining in affected patients, providing a structural explanation for the nutrient malabsorption that Aretaeus had described nearly eighteen centuries earlier.
