The situation changed decisively with Edward Jenner, a country physician practicing in Berkeley, Gloucestershire, who had heard the milkmaid lore since his own apprenticeship under the surgeon Daniel Ludlow in the 1760s. Jenner spent years collecting case histories, documenting farmers and dairy workers who reported prior cowpox infections and who appeared unharmed during subsequent smallpox outbreaks or who failed to develop the expected pustular response after deliberate inoculation with smallpox matter, a practice then known as variolation. On May 14, 1796, Jenner took material from a cowpox lesion on the hand of a milkmaid named Sarah Nelmes and inoculated an eight-year-old boy named James Phipps. After the boy recovered from a mild cowpox-like illness, Jenner subsequently inoculated him with smallpox matter and observed that no disease developed. Jenner repeated and documented similar experiments, and in 1798 he published his findings in a monograph titled "An Inquiry into the Causes and Effects of the Variolae Vaccinae."
Jenner coined the term "vaccine" from the Latin word for cow, "vacca," and the procedure he described became known as vaccination. Critics mocked the idea of introducing material from a diseased animal into human beings, and satirical cartoonists of the era depicted vaccinated individuals sprouting bovine features. Historically, physicians also debated whether the protective material maintained its efficacy as it was passed from person to person in serial arm-to-arm vaccination, and concerns arose about the consistency and purity of the vaccine material. The late nineteenth century saw a gradual transition toward more standardized preparations, a process that coincided with the broader professionalization of bacteriology and the work of figures such as Louis Pasteur, who extended the vaccination principle to other diseases and formally honored Jenner by applying the word "vaccine" universally to such protective preparations.
