Similarly, changes affecting the esophagus were documented by radiologists and gastroenterologists in the early twentieth century as barium swallow studies became available, revealing disordered motility in patients with scleroderma.
The particular clustering of calcinosis, vascular phenomena, esophageal involvement, and changes in the fingers was drawn together by various clinicians before Jacob Winterbauer published an influential 1964 paper that formally described what he called the CRST syndrome, using an acronym to capture the key features then recognized. The 'E' for esophageal dysmotility was incorporated shortly thereafter, completing the CREST formulation that became standard in clinical teaching for the latter decades of the twentieth century.
Immunological investigation in the second half of the twentieth century added another dimension to the historical understanding of this condition. Researchers including Eng Tan and his colleagues at the Scripps Research Institute identified antinuclear antibodies in patients with scleroderma and its variants during the 1970s and 1980s, and the anticentromere antibody pattern was specifically associated with the CREST presentation, offering serological evidence that aligned with the clinical distinctions physicians had been making on observational grounds for over a century.
