The modern delineation of cystic fibrosis as a distinct condition is most closely associated with the American pathologist Dorothy Hansine Andersen, who in 1938 published a landmark paper describing cystic fibrosis of the pancreas as a specific disease entity. Andersen conducted meticulous post-mortem examinations of children who had died of what was often recorded as celiac disease and identified a consistent pattern of pancreatic, pulmonary, and intestinal changes. Her work drew on dozens of cases and established the first coherent clinical and pathological picture of the condition. Andersen also proposed early on that the disease had a hereditary basis, a hypothesis that gained traction through subsequent family studies.

In the 1940s, Sydney Farber, a Boston pathologist, contributed additional observations and introduced the term 'mucoviscidosis,' emphasizing the abnormally thick mucus secretions as a central feature of the disease. This terminology reflected the prevailing focus on the sticky secretions that obstructed glands and ducts throughout the body. Researchers of that era debated whether the mucus abnormality was itself the root cause or merely a downstream consequence of some deeper defect.

The sweat electrolyte connection, long embedded in folklore, received scientific validation in the early 1950s through the work of Paul di Sant'Agnese, who discovered that children with the condition excreted abnormally high concentrations of salt in their sweat. This finding emerged partly from observations made during a New York heat wave when affected children showed unusual susceptibility to heat prostration. Di Sant'Agnese's discovery provided the physiological basis for the sweat test, which became the principal diagnostic tool for the condition throughout the latter half of the twentieth century.

The hereditary nature of the condition was clarified through mid-century genetic studies that established it as an autosomal recessive disorder, meaning that two copies of the altered gene were required for the condition to manifest. Carrier studies within families confirmed the inheritance pattern. The identification of the specific gene responsible, located on chromosome seven and designated CFTR, was achieved in 1989 through the collaborative work of Lap-Chee Tsui, Francis Collins, and Jack Riordan, marking one of the most celebrated achievements in the early years of molecular genetics and closing a chapter in the condition's long journey from folklore to genomic science.
