Historical Narrative

The earliest recorded references to deafness appear in ancient Egyptian papyri and in the legal codes of Mesopotamia, where deaf individuals were granted specific legal statuses distinct from those of hearing persons, suggesting that societies had long recognized deafness as a defined condition requiring formal acknowledgment. In ancient Greece, Aristotle articulated an influential and ultimately damaging position: he argued that hearing was the most important sense for the development of intelligence, and that those born deaf were incapable of reason and learning. This assertion carried enormous weight throughout the medieval period and into the Renaissance, shaping how physicians and philosophers approached the question of deaf education and the intellectual capacity of deaf individuals.

Galen of Pergamon, drawing on Hippocratic tradition, proposed that deafness arose from obstructions or injuries affecting the pathways between the ear and the brain. He distinguished between congenital deafness and deafness acquired through illness or trauma, a conceptual division that would persist in medical literature for over a thousand years. Medieval Arab physicians, including Ibn Sina, elaborated on Galenic frameworks in encyclopedic medical texts, attributing acquired deafness to humoral imbalances, excess moisture in the auditory canal, or injuries to what they termed the auditory nerve.

A significant turning point came in sixteenth-century Spain, where a Benedictine monk named Pedro Ponce de León began successfully teaching deaf students of noble families to speak, read, and write. His work directly challenged the Aristotelian consensus and demonstrated that deaf individuals were entirely capable of sophisticated communication and learning. Ponce de León left no detailed written account of his methods, but his successes were documented by contemporaries and inspired later educators.

In the seventeenth century, the English physician John Bulwer published works exploring the nature of deafness and the potential for manual communication, while the Spanish priest Juan Pablo Bonet published a more systematic manual alphabet method in 1620, providing one of the earliest documented instructional frameworks for educating deaf students. These developments represented a slow but meaningful shift away from purely medical fatalism toward educational intervention.

The eighteenth century brought further transformation. The French cleric Abbé Charles-Michel de l'Épée founded the first free public school for deaf students in Paris in the 1760s, developing a system of methodical signs that would later influence sign language development across Europe and the Americas. Around the same time, the Scottish physician William Cheselden and others were conducting anatomical investigations of the inner ear, attempting to correlate structural abnormalities with different presentations of hearing loss.

By the late eighteenth and early nineteenth centuries, physicians increasingly distinguished between what they called nerve deafness and deafness caused by obstruction or disease of the outer and middle ear, laying conceptual groundwork for later anatomical research. The German anatomist Alfonso Corti described the intricate structure of the cochlea's sensory apparatus in 1851, a discovery that fundamentally reoriented scientific understanding of how sound was processed, and the organ bearing his name became central to subsequent investigations into the physical basis of hearing loss.
