Historical Narrative

For the vast majority of recorded medical history, the constellation of symptoms now attributed to dementia with Lewy bodies was subsumed under sweeping, imprecise categories. Ancient Greek physicians, working within the humoral tradition, attributed disorders of memory and perception in older individuals to an excess of phlegm or black bile corrupting the brain's rational faculties. Galen of Pergamon, writing in the second century CE, described what he called morosis — a profound dulling of mental capacity in the aged — and attributed it to a cooling and drying of the brain substance over time. These frameworks persisted largely unchanged through the medieval period, when Islamic scholars such as Ibn Sina elaborated on Galenic theory in works like the Canon of Medicine, associating cognitive deterioration in the elderly with the natural exhaustion of innate vital heat.

The early modern period brought anatomical investigation to bear on mental disease, but the tools available remained too crude to distinguish between different causes of cognitive decline. Thomas Willis, the seventeenth-century English physician who coined the term neurology, performed careful dissections of brains from individuals who had suffered mental deterioration and noted structural abnormalities, but could not correlate specific lesions with specific clinical presentations in any reliable way.

The pivotal moment in the history of this condition arrived not through clinical observation but through the microscope. In 1912, the German neurologist Frederic Lewy, working in Alois Alzheimer's laboratory in Munich, was examining brain tissue from patients who had been diagnosed with Parkinson's disease when he observed unusual spherical inclusions — abnormal protein deposits — within nerve cells of the substantia nigra and other brainstem nuclei. Lewy published his findings but did not fully appreciate the broader significance of these structures, and the medical community paid them relatively little attention for decades.

Alois Alzheimer himself, in whose laboratory these observations were made, had already described in 1906 the plaques and tangles associated with the disease that would bear his name, and the early twentieth century's focus on those findings drew attention away from Lewy's inclusions. For much of the mid-twentieth century, elderly patients presenting with fluctuating cognition and visual disturbances were typically diagnosed with Alzheimer's disease or multi-infarct dementia, the two dominant diagnostic categories of the era.

Japanese neuropathologist Kenji Kosaka conducted the critical investigative work that would eventually reshape this history. Beginning in the 1970s and publishing landmark research through the 1980s, Kosaka systematically examined postmortem brain tissue and identified cases in which Lewy bodies — the very inclusions Frederic Lewy had noticed sixty years earlier — were distributed not only in the brainstem but diffusely throughout the cerebral cortex. Kosaka described this as diffuse Lewy body disease and was the first to propose it as a distinct clinicopathological entity separate from both Parkinson's disease and Alzheimer's disease.

International consensus on terminology and diagnostic criteria emerged slowly through the 1990s, with a landmark workshop held in Newcastle upon Tyne in 1996 producing the first widely adopted clinical criteria for what was then formally named dementia with Lewy bodies. This meeting represented the culmination of a process that had begun with a single researcher's microscopic observation in a Munich laboratory nearly a century before, and it transformed what had long been a pathologist's footnote into a recognized clinical diagnosis.
