Dermatophytosis, a fungal infection of the skin, hair, and nails caused by a group of parasitic fungi, was documented across ancient civilizations under a variety of descriptive names that reflected the visual patterns it produced on the body. For most of recorded history, healers understood the condition through humoral, miasmatic, or supernatural frameworks, with no concept of fungal organisms as causative agents. Among the earliest recorded descriptions of what historians of medicine have identified as likely dermatophytosis are references in ancient Egyptian medical texts and the Hebrew scriptures, where ringworm-like conditions of the scalp and skin were described under terms that translators have rendered variously as 'scall' or 'baldness.' The Mosaic Law recorded in Leviticus contained detailed instructions for priests to examine skin conditions and determine whether affected individuals should be isolated, reflecting an early recognition that certain skin ailments could spread through communities even if the mechanism of transmission was entirely unknown to those who documented it.

Greek physicians in the Hippocratic tradition described scaly, circular skin lesions under terms that translators have associated with ringworm, and Hippocratic writers attributed such conditions to imbalances in bile and other humors. The circular or ring-like pattern of the lesions captured the attention of ancient observers, and the Latin term 'tinea,' meaning moth or worm, entered the vocabulary of Roman medicine as healers speculated that an actual worm burrowed beneath the skin to produce the characteristic rings. This worm hypothesis, though entirely incorrect, proved remarkably persistent and shaped both nomenclature and treatment for well over a millennium.

Galen and other Roman-era physicians recommended various topical remedies for tinea conditions, including sulfur compounds, vinegar preparations, and resinous substances, some of which had genuine antifungal properties that neither the healers nor their patients could have understood in those terms. The empirical effectiveness of certain traditional remedies thus persisted across generations through practical observation rather than theoretical understanding.

Medieval Islamic physicians, drawing on Greek sources and their own clinical experience, wrote extensively about skin diseases. Ibn Sina's Canon of Medicine devoted considerable attention to conditions of the scalp and skin characterized by scaling and hair loss, recommending cleansing preparations and various plant-derived topical treatments. Al-Razi, known in the Latin West as Rhazes, produced detailed clinical descriptions of skin conditions that allowed later historians to identify probable cases of tinea capitis and other dermatophytic infections within his texts.

In medieval Europe, barbers and surgeons rather than university-trained physicians typically managed skin conditions, applying sulfur, tar, and other traditional preparations based on inherited craft knowledge. Outbreaks of scalp ringworm in particular were documented in orphanages, schools, and military camps, and some medieval accounts recorded the spread of the condition with a clarity that implied transmission between individuals, even without any concept of infectious agents.

The decisive transformation in understanding dermatophytosis came in the nineteenth century through the convergence of improved microscopy and a new scientific interest in the relationship between microorganisms and disease. Johann Lukas Schönlein, the German physician, is credited with the first identification of a fungal organism as the cause of a skin condition when in 1839 he described the fungus responsible for favus, a severe scalp infection. His student David Gruby expanded on this work substantially in the 1840s, describing multiple fungal species associated with different tinea conditions and demonstrating their role under the microscope with unprecedented clarity. Gruby's work, conducted largely in Paris, represented a foundational contribution to the new science of medical mycology.

Raymond Sabouraud, the French dermatologist working at the turn of the twentieth century, produced the most comprehensive systematic classification of dermatophytes achieved up to that point, developing culture media that allowed the organisms to be grown and studied in laboratory conditions, and publishing his landmark atlas of tinea diseases that became a standard reference for dermatologists across the Western world.
