Diabetes mellitus was a condition recognized by healers across multiple ancient civilizations, characterized in historical records primarily by the production of large quantities of sweet-tasting urine and a progressive wasting of the body. For most of recorded medical history, physicians had no means of addressing the underlying cause of the disease and focused their efforts on dietary regimens, herbal preparations, and supportive care that could at best slow the patient's decline. The twentieth century brought the most dramatic transformation in the medical understanding of the condition, when researchers isolated the pancreatic substance that proved central to the body's handling of sugar.

Historical Narrative

Among the earliest written references to a condition resembling diabetes mellitus were those found in the Ebers Papyrus, an Egyptian medical document dating to approximately 1550 BCE, which described a state of excessive urination and recommended remedies drawn from plant and animal sources. Ancient Indian physicians of the Ayurvedic tradition, including Charaka and Sushruta writing in the early centuries of the common era, described what they called Madhumeha, or honey urine, and noted that it afflicted the wealthy who consumed rich foods, as well as those with hereditary predisposition. Sushruta further distinguished between two forms of the condition based on the patient's physical constitution, a distinction that would echo in later medical classifications.

In ancient Greece, the physician Aretaeus of Cappadocia, writing in the second century CE, produced what many historians consider the most vivid classical description of the disease. He coined the term diabetes, from the Greek word for siphon, capturing the relentless passage of fluid through the body, and described the condition as a melting down of the flesh and limbs into urine. Galen of Pergamon also wrote of the disease but considered it a disorder of the kidneys, a view that shaped European medical thinking for centuries.

Medieval Islamic physicians made significant contributions to the understanding of diabetes. Avicenna, writing in his Canon of Medicine in the eleventh century, described the abnormal appetite, weakness, and sweet urine associated with the condition, and proposed treatments involving fenugreek, lupine seeds, and other botanical preparations. He also noted the tendency toward gangrene in affected patients, a clinical observation that physicians across cultures had recorded with grim regularity.

The seventeenth century brought a pivotal observation when the English physician Thomas Willis confirmed through taste that the urine of affected patients was sweet, describing it as wonderfully sweet, as if it were imbued with honey or sugar. Matthew Dobson later demonstrated in the eighteenth century that the sweet substance in the urine of diabetic patients was in fact sugar, and that the same substance could be detected in elevated quantities in the blood, shifting understanding of the disease from a purely renal disorder toward a systemic one.

The nineteenth century saw Johann Peter Frank and others attempt to distinguish between diabetes mellitus and diabetes insipidus as separate conditions, while Claude Bernard's experimental work in the 1850s revealed the liver's role in producing and releasing glucose into the bloodstream, providing a new mechanistic basis for theorizing about the disease. The discovery of the pancreas's role came through the work of Oskar Minkowski and Joseph von Mering in 1889, who found that removal of the pancreas in dogs produced a state resembling diabetes, pointing investigators toward the organ as the seat of whatever regulatory function was failing.

The critical breakthrough arrived in 1921 and 1922, when Frederick Banting and Charles Best, working in the laboratory of John Macleod at the University of Toronto, succeeded in isolating an active pancreatic extract they called insulin. James Collip refined the purification process, and the substance was first administered to a human patient, Leonard Thompson, in January 1922, transforming diabetes mellitus from an almost universally fatal condition into a manageable one for the first time in history.
