Hippocrates himself wrote in the fifth century BCE of throat ulcerations accompanied by a suffocating membrane, though his understanding attributed such afflictions to environmental imbalances and disturbances in the bodily humors rather than to any transmissible agent. Syrian and Egyptian sources from the same era catalogued similar lethal throat conditions, suggesting the disease held a persistent and feared presence across the ancient Mediterranean world.

Throughout the medieval period, epidemic outbreaks of what historians now believe was diphtheria were recorded across Europe and the Middle East, often described under names emphasizing the characteristic membranous throat obstruction. Arabic physicians, including those working within the Galenic tradition, interpreted such episodes through frameworks of corrupt air and constitutional weakness. The disease periodically devastated entire communities, with chroniclers noting its particular cruelty toward infants and young children.

The sixteenth and seventeenth centuries witnessed some of the most catastrophic diphtheria epidemics in recorded European history. Spanish physicians in the late sixteenth century documented outbreaks they called "el garotillo," meaning the strangler, a name that evoked the suffocating death the disease produced. Guillaume de Baillou, a French physician working in Paris in the late sixteenth century, provided meticulous clinical descriptions of epidemic throat disease that historians later recognized as diphtheria, making him among the first to systematically observe its epidemic character.

The naming of diphtheria as a distinct disease entity came in 1826, when French physician Pierre Bretonneau formally distinguished it from other throat conditions and coined the term from the Greek word for leather, referring to the tough membrane he observed. Bretonneau's careful clinical work established diphtheria as a specific disease rather than a symptom of various ailments, representing a significant conceptual advance. His pupil Armand Trousseau further elaborated clinical understanding of the condition in mid-nineteenth century Paris.

The bacteriological revolution transformed the understanding of diphtheria entirely. Edwin Klebs identified the responsible bacterium in 1883, and Friedrich Loeffler cultured it in 1884, together lending their names to what became known as the Klebs-Loeffler bacillus, now called Corynebacterium diphtheriae. The discovery that the bacterium produced a toxin responsible for the disease's systemic damage came through the work of Émile Roux and Alexandre Yersin at the Pasteur Institute in 1888, opening entirely new possibilities for intervention.

Emil von Behring and Shibasaburo Kitasato demonstrated in 1890 that injecting progressively larger doses of diphtheria toxin into animals produced a protective antitoxin in their blood, work that earned von Behring the first Nobel Prize in Physiology or Medicine in 1901. The dramatic public demonstration of antitoxin therapy in hospitalized children in Paris in the early 1890s represented a pivotal moment in the history of medicine. The later development of toxoid immunization by Gaston Ramon in the 1920s laid the groundwork for systematic vaccination campaigns that would eventually reduce diphtheria from a mass killer to a rarity across much of the world.
