In the late nineteenth and early twentieth centuries, physicians debated its origins vigorously, proposing explanations ranging from maternal tuberculosis and alcoholism to parental age, syphilis, and hereditary degeneration. Institutionalization was the standard societal response across Europe and North America, and prognoses offered in medical literature were uniformly bleak.

The mid-twentieth century brought a transformation on multiple fronts. In 1959, the French geneticist Jérôme Lejeune, working with Marthe Gautier and Raymond Turpin in Paris, announced the discovery that the condition was associated with the presence of an extra chromosome at the twenty-first pair, establishing trisomy 21 as its genetic basis. This finding was among the first demonstrations that a human condition could be traced to a chromosomal anomaly and inaugurated a new era in human genetics. Lejeune had hoped the discovery would lead to treatment, a goal that eluded him throughout his career.

Simultaneously, advocacy movements led by parents and educators challenged the institutional model, pressing for education, community inclusion, and recognition of individual potential.
