Historical Narrative

The earliest systematic written account of epilepsy appears in the Babylonian medical text known as the Sakikku, compiled around 1050 BCE but incorporating material from considerably earlier periods. Babylonian physician-scholars described multiple distinct types of seizure with clinical precision that would not be matched for millennia, attributing the condition to the action of specific demons, particularly the god Sin, and prescribing ritual exorcisms performed by the āšipu alongside herbal preparations administered by the āsû, the more practically oriented healer. The association between epilepsy and supernatural possession was nearly universal across ancient cultures, appearing in ancient Chinese, Indian, Egyptian, and Hebrew sources, and reflected the terrifying and inexplicable nature of sudden loss of consciousness and convulsive movement.

Ancient Greek medicine produced the most consequential early reinterpretation of epilepsy. The Hippocratic text On the Sacred Disease, written around 400 BCE, mounted a sustained and historically revolutionary argument against the supernatural explanation of epilepsy, insisting that the condition had a natural cause rooted in the brain and was no more sacred than any other illness. The author argued that heredity and the constitution of the phlegm contributed to the condition, and that it arose from an obstruction of air flowing through the vessels of the brain. This naturalistic turn was foundational for subsequent Western medical thinking, even as popular and religious associations with the divine or demonic persisted for centuries afterward.

Galen expanded Hippocratic theory, classifying epilepsy according to the location of its origin in the brain, heart, or stomach, a tripartite scheme that influenced medical thinking well into the early modern period. Roman physicians treated epilepsy with bloodletting, dietary restrictions, and an array of substances including mistletoe, human skull, and gladiator blood, the last of which was consumed in the belief that the vitality of a fallen warrior could counteract the convulsive crisis.

Medieval European medicine inherited both the naturalistic Galenic tradition and the persistent popular belief in demonic causation. Church authorities sometimes performed exorcisms on people with epilepsy while learned physicians simultaneously applied Galenic remedies. The condition acquired the name morbus comitialis in Rome, because a seizure was believed to render a public assembly invalid, and morbus herculeus, associating it with the hero Hercules, among many other historical names.

The early modern period saw gradual refinement of clinical description. Thomas Willis, the seventeenth-century English physician whose anatomical work gave neurology much of its vocabulary, produced detailed accounts of epilepsy in his Cerebri Anatome of 1664 and Pathologiae Cerebri of 1667, attempting to locate its mechanism in the animal spirits he believed circulated through the nervous system.

The most transformative chapter in epilepsy's scientific history began with John Hughlings Jackson in the nineteenth century. Jackson, working at the National Hospital for the Paralysed and Epileptic in London, developed a rigorous observational approach that distinguished different types of seizure according to their progression and their anatomical implications, establishing that focal seizures could point to specific regions of the brain. His theoretical framework, which conceptualized seizures as the result of occasional, sudden, excessive, rapid, and local discharges of gray matter, formed the conceptual foundation on which twentieth-century neurophysiology built. Jackson's colleague and contemporary David Ferrier complemented this work through experimental cortical mapping, helping to confirm the localizationist principles that Jackson had inferred from clinical observation.

The introduction of potassium bromide as the first pharmaceutical agent used specifically for epilepsy, pioneered by Sir Charles Locock in 1857, marked the beginning of the pharmacological era in epilepsy treatment and closed a chapter in which medicine had been largely powerless against the condition.
