Ancient healers connected prominent eyes to humoral excess, structural abnormality, or constitutional character, while early modern physicians began to associate the sign with specific systemic diseases involving the neck and nervous system. The recognition that exophthalmos was a cardinal feature of thyroid-related disease came only through the careful clinical observations of physicians working in the late eighteenth and early nineteenth centuries.

Historical Narrative

Ancient Greek and Egyptian medical texts included observations about abnormally prominent eyes in discussions of constitutional types and pathological states. Hippocratic writings noted that certain individuals possessed eyes that appeared to bulge forward, and this characteristic was sometimes associated with particular temperamental and health tendencies within humoral theory. The bulging eye was also depicted in ancient sculpture and funerary art across Mediterranean and Near Eastern cultures, suggesting that the sign was familiar enough to be rendered with artistic intention.

Galen of Pergamon discussed orbital and ocular anatomy in considerable detail and was aware of pathological protrusions of the eye, attributing such conditions to accumulations of fluid or humor behind the globe, or to abnormal swellings of orbital tissues. His anatomical work on the eye and its surrounding structures was preserved and elaborated in Islamic medical literature, where Ibn Sina in the Canon of Medicine discussed swellings and growths within the orbit that could displace the eyeball forward, cataloguing these among disorders of the ocular region.

Medieval European physicians largely followed Galenic and Avicennian frameworks in interpreting orbital protrusion as a local anatomical phenomenon related to abscess, tumor, or humoral accumulation in the region behind the eye. Surgical texts from the medieval period occasionally referenced attempts to address such protrusions through drainage of orbital collections, and the condition was distinguished from simpler surface abnormalities of the eye itself.

The pivotal transformation in understanding came with a series of clinical descriptions published in the late eighteenth and early nineteenth centuries that linked prominent eyes to enlargement of the thyroid gland in the neck and to a constellation of other signs including rapid heart action and tremor. Caleb Hillier Parry, a Bath physician, compiled observations of patients exhibiting this combination of findings beginning in the 1780s, though his account was not published until 1825, after his death. Simultaneously and independently, Robert James Graves in Dublin published his famous description in 1835 of patients with palpitation, thyroid swelling, and exophthalmos, a report that gave his name to the condition in British and American medical tradition. Karl Adolph von Basedow in Germany published a similar clinical account in 1840, and his name became attached to the syndrome in continental European medical usage.

The recognition of what came to be called Graves' disease or Basedow's disease represented a major reorientation of thinking about exophthalmos: the prominent eye was no longer primarily a local orbital phenomenon but a systemic sign linked to dysfunction of the thyroid gland. This shift prompted subsequent investigators to explore the relationship between the thyroid and both the cardiovascular system and the nervous system throughout the mid and late nineteenth century.

Moritz Schiff's experimental work on thyroid extirpation in animals during the 1850s contributed to the emerging understanding that the thyroid gland exerted systemic effects, though the nature of those effects remained obscure. The isolation and characterization of thyroid secretions as chemical messengers with measurable physiological actions was a project that occupied physiologists and clinicians through the latter decades of the nineteenth century and into the early twentieth, gradually replacing humoral and neurological theories of thyroid-related exophthalmos with endocrine ones.
