The earliest references to a condition resembling glaucoma appear in ancient Greek medical writing, where the term glaukos — meaning a blue-gray or sea-green color — was applied to eyes that had taken on a distinctive clouded or discolored appearance accompanying blindness. Hippocratic writers used the term glaukoma to describe a loss of sight in older individuals accompanied by a grayish pupil, though scholars have long debated whether these ancient descriptions referred to what is now classified as glaucoma, cataracts, or other causes of visual clouding.

Galen perpetuated the Greek terminology and interpreted the grayish discoloration as a pathological humor settling in the lens or the fluid spaces of the eye, consistent with his humoral framework for all disease. This interpretation effectively conflated what later physicians would distinguish as separate conditions, and Galenic descriptions of glaukoma encompassed multiple forms of vision loss under a single imprecise label.

Arabic physicians of the medieval period engaged extensively with ophthalmology, and figures such as Hunayn ibn Ishaq, the ninth-century scholar who translated Greek medical texts into Arabic and Persian and wrote his own influential treatises on the eye, provided some of the most detailed early medieval descriptions of ocular anatomy and disease. Ibn al-Haytham's work on optics in the eleventh century deepened the broader intellectual framework surrounding vision, though the specific mechanisms of glaucomatous damage remained as obscure to Arabic physicians as to their Greek predecessors.

European Renaissance anatomists, including Bartolomeo Eustachi and later Andreas Vesalius, contributed improved anatomical descriptions of the eye, but the concept of intraocular pressure and its relationship to disease remained absent from medical thinking. Surgeons of the sixteenth, seventeenth, and eighteenth centuries occasionally performed couching operations — a technique of displacing the lens with a needle that dated to ancient India and was transmitted through Arabic surgical traditions — for presumed lens-related blindness, sometimes inadvertently operating on glaucoma cases with no beneficial result.

The pivotal transformation in understanding came in the nineteenth century. In 1851, Hermann von Helmholtz invented the ophthalmoscope, an instrument that allowed direct visualization of the interior of the living eye including the optic disc, enabling physicians to observe the characteristic excavation of the optic nerve head that accompanies glaucoma. The following decade brought equally consequential advances when Albrecht von Graefe, the preeminent German ophthalmologist of the era, distinguished between different forms of the condition and established the critical role of elevated intraocular fluid pressure in causing blindness.

Von Graefe introduced iridectomy — surgical removal of a portion of the iris — as a treatment for acute angle-closure presentations, and his systematic clinical observations helped define glaucoma as a pressure-related disorder rather than a humoral or purely degenerative one. The development of tonometry, the measurement of intraocular pressure through the eye's surface, by Adolph Weber and later refined by Hjalmar Schiotz in the late nineteenth and early twentieth centuries gave clinicians a quantitative tool that was entirely absent from all preceding eras of medical practice, fundamentally closing the long era in which glaucoma could only be diagnosed after blindness had already advanced.
