For millennia, the condition was attributed to causes ranging from bad water to supernatural forces, long before the gland itself was even named. Among the earliest documented references to goitre appeared in Chinese medical texts dating to around 2700 BCE, where the swollen neck condition was described and treated with seaweed and burnt sponge — substances now understood to be rich in iodine, though ancient practitioners had no knowledge of that element. Indian Ayurvedic texts from roughly the same era also catalogued the condition under the term 'galaganda,' offering herbal remedies and dietary adjustments believed to reduce the swelling. Egyptian physicians referenced neck swellings in papyri, and artistic depictions on ancient tomb walls have been interpreted by some historians as representations of goitrous individuals, suggesting the condition was common enough in the Nile Valley to be considered worthy of record.
Greek and Roman physicians devoted considerable attention to the swollen neck. Hippocratic writings in the fifth century BCE grouped visible neck swellings under broader categories of tumour and excess humour, attributing their origin to an imbalance of phlegm. The Roman physician Pliny the Elder noted in his 'Naturalis Historia' that alpine and mountain-dwelling populations were disproportionately afflicted, an observation that proved remarkably accurate in light of later scientific understanding, though Pliny himself connected it to the quality of local water and air rather than mineral content. Celsus and Galen elaborated on surgical approaches to large neck swellings, and Galen's humoral framework dominated medical interpretation of the condition for over a thousand years.
During the medieval period, European healers continued to rely on burnt sponge and seaweed-based remedies passed down from antiquity, often administered within religious and monastic medical traditions. Islamic physicians of the ninth through eleventh centuries, particularly Ibn Sina, whose 'Canon of Medicine' synthesised Greek and Arabic learning, described cervical swellings in careful anatomical terms and recommended thyroid-adjacent treatments. The anatomist Andreas Vesalius in the sixteenth century produced more precise descriptions of neck anatomy, though the specific gland that would become known as the thyroid was only formally named and described in greater anatomical detail by Thomas Wharton in his 1656 work 'Adenographia,' where he coined the term 'thyroid' from the Greek word for shield.
The eighteenth century brought more systematic geographic surveys of goitre prevalence, with Swiss and French physicians documenting its extraordinary frequency in alpine regions. Physicians including Bernard Courtois, who isolated iodine in 1811, and later Jean-Baptiste Boussingault, began to connect iodine-deficient environments with high rates of the condition. The French physician Gaspard Coindet conducted early experiments in the 1820s administering iodine to goitrous patients, achieving notable reductions in swelling, though his methods were criticised as imprecise and sometimes harmful in excess.
Surgical intervention for goitre had a long and dangerous history. Before antiseptic technique and anaesthesia, thyroid surgeries carried extraordinarily high mortality rates. The Swiss surgeon Theodor Kocher transformed this landscape in the latter half of the nineteenth century, refining surgical techniques for thyroid removal to the point that his mortality rates dropped dramatically from those of his predecessors. Kocher was awarded the Nobel Prize in Physiology or Medicine in 1909, in part for this work, cementing the thyroid's place at the centre of a new era of endocrinological inquiry.
