Before the twentieth century, paralytic illnesses that ascended from the limbs toward the trunk were categorized under broad and imprecise labels. Landry's ascending paralysis, named for the French physician Jean-Baptiste Octave Landry who published a careful case series in 1859, described a pattern of progressive limb weakness that historical scholars have retrospectively associated with what later became known as Guillain–Barré syndrome. Landry identified ten cases, including his own observations and those drawn from earlier literature, and described the ascending nature of the weakness with notable clarity. However, he possessed no means of examining the cerebrospinal fluid or confirming the absence of fever that would later serve to distinguish the syndrome from infectious and inflammatory competitors.

Throughout the second half of the nineteenth century, European neurologists accumulated case reports of acute flaccid paralysis under various nomenclatures. The German neurological tradition, represented by figures such as Wilhelm Erb, contributed careful electrodiagnostic and reflex observations, noting the characteristic loss of tendon reflexes in certain paralytic conditions that did not fit established diagnoses of spinal cord disease or poliomyelitis. The development of the lumbar puncture technique by Heinrich Quincke in 1891 created a new investigative tool that would prove essential to the eventual definition of the syndrome.

The decisive publication appeared in 1916 in the Bulletin de la Société Médicale des Hôpitaux de Paris, authored by Georges Guillain, Jean Alexandre Barré, and André Strohl. Working as military physicians during the First World War, the three described two French soldiers with areflexia, limb weakness, and a striking cerebrospinal fluid finding: elevated protein levels in the absence of increased cell count, a pattern they termed albuminocytologic dissociation. This specific laboratory signature distinguished the condition from meningitis and poliomyelitis and gave neurologists a concrete diagnostic criterion for the first time. Strohl contributed electrodiagnostic observations to the paper but was not ultimately included in the eponym that followed, an omission later noted by historians of neurology.

The eponym Guillain–Barré became standard in French and then international neurological literature, though German-speaking physicians for some time used the designation Landry–Guillain–Barré to acknowledge the earlier contribution. Throughout the 1920s and 1930s, additional cases were reported across Europe and North America, and neurologists debated whether the condition represented a single entity or a heterogeneous group of paralytic disorders. The infectious trigger for the syndrome attracted speculation early, as many patients recalled a preceding illness, but the mechanism remained obscure.

Mid-twentieth-century research brought increased understanding of the nerve pathology involved. Neuropathologists examining post-mortem tissue described inflammatory changes to the myelin sheath surrounding peripheral nerves, leading researchers to situate the condition within the emerging category of demyelinating diseases. The 1976 swine influenza vaccination campaign in the United States became a watershed moment in the condition's epidemiological history, as surveillance data suggested a statistical association between the vaccine and a rise in cases, prompting suspension of the program and renewed scientific investigation into the syndrome's immunological underpinnings. This episode accelerated research that would eventually characterize the condition as an immune-mediated process directed against peripheral nerve components.
