The history of the aneurysm, a pathological bulging or dilation of an artery, stretches back to antiquity, where healers encountered dramatic pulsating masses in patients and recorded their observations without understanding the underlying vascular mechanism. The word aneurysm derives from the ancient Greek meaning a widening, and it appears in the writings of Galen of Pergamon in the second century CE, where he described both traumatic and spontaneous varieties and distinguished aneurysm from hematoma. Earlier references appear in the works attributed to the Byzantine compiler Aetius of Amida and in the corpus of writings associated with Antyllus, a Greek surgeon of the second or third century CE who devised one of the earliest recorded surgical approaches to the condition, involving proximal and distal ligation of the affected vessel followed by incision and evacuation of the contents.
Medieval Islamic medicine preserved and extended Greek knowledge of aneurysms. Albucasis, the Moorish physician writing in eleventh-century Andalusia, described surgical approaches to pulsating tumors in his influential encyclopedia Al-Tasrif, which became a standard reference in European medical schools after its translation into Latin. Medieval European physicians, however, largely retreated from surgical intervention and relied on conservative management, reflecting both the high mortality of operations and the philosophical conservatism of scholastic medicine.
The Renaissance reinvigorated anatomical study and brought new attention to vascular pathology. Ambroise Paré, the French military surgeon of the sixteenth century who elevated surgery from a craft to a respected art, wrote extensively about aneurysms encountered in battlefield casualties and is credited with reintroducing ligation as a treatment approach. His careful observations of gunshot wound complications helped establish that aneurysms could form at the site of traumatic arterial injury, an important distinction from spontaneous varieties.
The connection between syphilitic infection and aortic aneurysm was established gradually through the eighteenth and nineteenth centuries. Clinicians noticed that patients with known syphilis disproportionately developed aneurysms of the thoracic aorta, though the mechanism was debated. Giovanni Maria Lancisi published detailed observations on aortic disease in the early eighteenth century. By the mid-nineteenth century, anatomical pathologists including Carl Rokitansky in Vienna were systematically classifying aneurysms according to their pathological features and distinguishing syphilitic aortitis as a specific entity responsible for a large proportion of aortic aneurysms seen in autopsy rooms.
Antonio Scarpa, the Italian anatomist, published a celebrated monograph on aneurysms in 1804 that synthesized contemporary knowledge and provided meticulous illustrations. The nineteenth century also saw the first serious attempts to treat aortic aneurysms by inducing clotting within the sac, including the method of introducing wire into the aneurysm, credited to Charles Moore working in London in 1864, and the application of electrical current, a technique explored by Corradi. These approaches had limited success but represented the first systematic attempts to apply emerging physiological knowledge to aneurysm management. Rudolph Matas, working in New Orleans at the turn of the twentieth century, developed the endoaneurysmorrhaphy operation and became one of the most celebrated vascular surgeons of his era, his work representing the culmination of centuries of effort to find a surgical answer to this dangerous condition.
