Among the earliest written references to what historians believe was cholera appear in ancient Sanskrit texts from the Indian subcontinent, dating as far back as 400–500 BCE, describing a sudden and catastrophic illness associated with water sources in the Ganges Delta region. Ancient Greek and Roman physicians documented similar episodes, though they lacked any framework to distinguish cholera from other acute illnesses and generally attributed mass sickness to corrupted air, divine punishment, or imbalances in the four humors.

For centuries, the dominant Western medical tradition, inherited from Galen and Hippocrates, held that diseases like cholera arose from 'miasmas,' meaning poisonous vapors emanating from rotting organic matter, swamps, and filth. This theory was not merely folk superstition — it was the scholarly consensus of trained physicians well into the 19th century. When cholera exploded out of the Bengal region and swept westward in a series of devastating pandemics beginning around 1817, physicians across Europe and the Americas scrambled to explain it within this miasmatic framework.

The first pandemic (1817–1824) stunned the world as the disease traveled trade routes into Persia, Arabia, and East Africa. A second pandemic (1829–1851) reached Europe and the Americas for the first time, killing tens of thousands in cities wholly unprepared for its speed. During this era, treatments prescribed by physicians of good standing included bloodletting, purging with calomel (a mercury compound), opium, and the application of mustard plasters — interventions grounded entirely in humoral medicine and miasmatic assumptions.

The pivotal intellectual turning point came during the 1854 London outbreak centered in the Soho district. John Snow, a physician already skeptical of miasma theory, meticulously mapped cholera cases against the neighborhood's water pumps and identified the Broad Street pump as the epicenter of transmission. His careful geographic and statistical analysis, documented in his landmark work 'On the Mode of Communication of Cholera,' was among the first demonstrations of epidemiological field investigation in history. Crucially, Snow's contemporaries were largely unconvinced at the time; miasma theory retained institutional dominance for another decade.

Almost simultaneously, the Italian physician Filippo Pacini identified a comma-shaped microorganism in the intestinal tissue of cholera victims in 1854, publishing his findings but receiving little attention. It was not until Robert Koch independently isolated and described Vibrio cholerae in 1883 during an outbreak investigation in Egypt and India that the scientific community broadly accepted the bacterial origin of the disease. Koch's announcement transformed cholera from a mystery of poisoned air into a problem of contaminated water and sanitation — a reframing that permanently altered how cities were built and governed.

The history of cholera is therefore also the history of municipal water systems, sewage infrastructure, and the political battles over public health authority that defined the late 19th century. Cities that built comprehensive sanitation works saw dramatic reductions in epidemic mortality, validating Snow's and Koch's frameworks in practice long before every detail of bacteriology was fully understood.
