Hepatitis, meaning inflammation of the liver, has a recorded history spanning at least two and a half millennia, with epidemic jaundice described by ancient writers across multiple civilizations and later devastating military campaigns throughout modern history. For most of recorded history, physicians understood jaundice as a symptom of humoral imbalance rather than as evidence of viral infection, and it was only the rigorous epidemiological and virological work of the 20th century that revealed hepatitis to encompass several distinct infectious diseases with fundamentally different transmission routes. The discovery and cataloguing of the hepatitis viruses across the second half of the 20th century stands as one of the great achievements of modern infectious disease research.

Historical Narrative

The Babylonian Talmud contains what is considered one of the earliest references to infectious jaundice, advising communities to isolate individuals with yellowed skin, suggesting that ancient observers recognized the contagious nature of at least some forms of hepatitis without understanding its mechanism. Hippocrates described epidemic jaundice in his writings, and outbreaks of what was almost certainly hepatitis appear in accounts of military campaigns, religious pilgrimages, and urban epidemics throughout the ancient and medieval world. The yellowing of the skin and eyes, which we now understand as jaundice from liver inflammation and bile duct disruption, was interpreted by Galenic physicians as an overflow of yellow bile, one of the four humors, into the blood.

Medieval Islamic physicians, including Ibn Sina, who wrote extensively about liver diseases in his Canon of Medicine in the 11th century, distinguished various forms of jaundice by their presumed humoral origin and associated constitutional features. European medieval physicians followed similar frameworks, and the condition variously called 'epidemic jaundice,' 'campaign jaundice,' or 'catarrhal jaundice' appeared repeatedly in accounts of Crusader armies, early modern military forces, and civilian populations during periods of famine and crowding.

The 18th and early 19th centuries produced more systematic clinical observation of epidemic jaundice, with German and Italian physicians documenting outbreaks and debating whether the condition was contagious or arose spontaneously from miasmas and poor conditions. A notable 1885 outbreak among German shipyard workers who had received a smallpox vaccination made from human lymph material provided early epidemiological evidence for a blood-borne transmission route, though the significance of this observation was not fully appreciated at the time.

World War II proved to be a pivotal episode in hepatitis research. Massive outbreaks among military personnel on multiple fronts, partly traced to contaminated yellow fever vaccines administered to millions of soldiers, forced the American and British military medical establishments to take the epidemiology of hepatitis seriously. Researchers including Frederick MacCallum of Britain and John Rodman Paul of Yale University worked during and after the war to distinguish two epidemiologically distinct forms of hepatitis: one spread through contaminated food and water, which MacCallum designated hepatitis A, and one spread through blood and other bodily fluids, designated hepatitis B, a terminology formally adopted in 1947.

The identification of the actual viral agents took considerably longer. Baruch Blumberg's discovery of the Australia antigen in 1965, initially in blood samples from an Aboriginal Australian, led to the identification of the hepatitis B surface antigen and the virus behind it, transforming hepatitis B from an epidemiological abstraction into a concrete, detectable pathogen. Blumberg received the Nobel Prize in Physiology or Medicine in 1976. The hepatitis A virus was directly visualized by Stephen Feinstone using electron microscopy in 1973. The discovery that a third major form of hepatitis existed, caused by yet another virus resistant to earlier detection methods, led to the identification of the hepatitis C virus by Michael Houghton and colleagues at Chiron Corporation in 1989, work recognized with the 2020 Nobel Prize in Physiology or Medicine shared by Houghton, Harvey Alter, and Charles Rice.
