Evidence of osteoporotic bone changes has been identified in Egyptian mummies and skeletal remains from ancient Greece and Rome, with paleopathological analysis revealing the tell-tale thinning of cortical bone and increased trabecular porosity in individuals from these ancient populations. Ancient physicians, however, had no way to observe these internal changes in living patients, and bone fragility in elderly people was generally accepted as an unremarkable feature of aging rather than a disease process requiring explanation.

Ancient and medieval medical traditions did not produce a concept corresponding to osteoporosis in any recognizable form. Greco-Roman medical writers understood bones as among the driest and most stable of the body's structures, making them somewhat peripheral to the humoral reasoning that animated most ancient disease theory. When elderly individuals suffered bone fractures, the cause was attributed to the natural drying and weakening that humoral theory associated with old age, a depletion of vital moisture that was seen as philosophically inevitable rather than medically addressable.

The anatomical revolution of the sixteenth century, catalyzed most famously by Andreas Vesalius whose landmark illustrated anatomy De Humani Corporis Fabrica was published in 1543, transformed European understanding of the skeleton through systematic dissection and detailed illustration. Yet even this revolution did not produce a concept of pathological bone loss because anatomists were primarily mapping structure rather than investigating disease processes within bone tissue.

The microscopic study of bone became possible in the seventeenth century with the advent of early microscopy. Antonie van Leeuwenhoek examined bone tissue microscopically in the late seventeenth century and described its internal structure, and the English physician Clopton Havers published a systematic microscopic study of bone in 1691, describing the canal systems within bone that subsequently bore his name as Haversian canals. These observations laid groundwork for understanding bone as a dynamic living tissue rather than an inert structural material, a conceptual shift that was necessary before pathological bone changes could be meaningfully investigated.

The development of X-ray technology following Wilhelm Röntgen's discovery in 1895 transformed the practical ability to observe bone density in living patients for the first time in history, though early radiological assessment of bone loss was crude and highly subjective. The critical figure in establishing osteoporosis as a formal medical and scientific category was the American endocrinologist Fuller Albright, who published landmark research in the 1940s arguing that osteoporosis was a systemic disease of impaired bone formation rather than simply an acceleration of normal aging. Albright drew particular attention to the relationship between osteoporosis and estrogen deficiency following menopause, work he conducted partly while himself suffering from Parkinson's disease, and his classification framework shaped medical understanding of the condition for decades.

Albright's work was built upon by subsequent researchers who developed increasingly precise methods for measuring bone mineral density, eventually leading to the radiological techniques that made population-level study of bone loss scientifically feasible in the latter half of the twentieth century. The World Health Organization's formal definition of osteoporosis using standardized bone density measurements was established only in 1994, making osteoporosis one of the most recently formalized major disease categories in mainstream medicine despite its ancient biological existence.
