Historical Narrative

Ancient Greek and Roman physicians described stomach ailments involving pain and digestive distress, and Galen wrote extensively about conditions he attributed to excess acid or bilious humors corroding the stomach lining. However, the precise anatomical entity of a gastric or duodenal ulcer was not clearly described until the early modern period, when autopsies allowed physicians to observe the stomach's interior in detail. Giovanni Battista Morgagni, the 18th-century Italian anatomist who essentially founded the discipline of pathological anatomy, documented gastric ulcers in post-mortem studies and helped establish their physical reality as lesions distinct from broader stomach disease.

By the 19th century, French and German physicians refined the anatomical picture considerably. Jean Cruveilhier, a French pathologist, provided detailed descriptions of gastric ulcers in his illustrated anatomical atlas published in the 1830s, and the condition bore his name for a period in French medical literature. German physician Karl Schwarz articulated in 1910 the famous dictum 'no acid, no ulcer,' which would shape ulcer treatment philosophy for much of the 20th century and made acid suppression the logical therapeutic goal.

The early 20th century brought the widespread adoption of the psychological and stress model of ulcer disease. Influential American physiologist Walter Cannon had demonstrated in the early 1900s that emotional states affected gastric function in animals, and this finding was extrapolated into a cultural narrative in which the driven, anxious businessman became the archetypal ulcer patient. This model was reinforced by observations during World War II that civilian populations under bombing stress in London appeared to have elevated rates of perforated ulcers, a finding later disputed by more careful epidemiological analysis.

The pharmaceutical industry developed a series of increasingly effective acid-suppressing drugs across the mid-to-late 20th century, particularly the histamine H2 receptor antagonists pioneered by James Black and his colleagues at SmithKline in the 1970s. Cimetidine, launched in 1976, became one of the best-selling drugs in the world and appeared to validate the acid model entirely by demonstrating that reducing acid production led to ulcer healing. The approach was further refined with the development of proton pump inhibitors in the 1980s.

The entire conceptual edifice shifted dramatically in 1984 when Australian physician Barry Marshall, working with pathologist Robin Warren, announced evidence that a spiral-shaped bacterium, subsequently named Helicobacter pylori, was consistently found in the stomachs of ulcer patients. Warren had first noticed the organism in 1979. Their finding was met with deep skepticism from a medical establishment committed to the acid and stress paradigm. To demonstrate bacterial causation more forcefully, Marshall famously ingested a solution containing the bacterium himself and developed gastric inflammation, an act of self-experimentation that has become legendary in medical history. The scientific community gradually accepted their findings across the late 1980s and 1990s, and Marshall and Warren were awarded the Nobel Prize in Physiology or Medicine in 2005 for their discovery, one of the clearest examples of a dominant medical paradigm being overturned by a single decisive body of evidence.
