The historical record of plague stretches back to antiquity, though scholars debate which early epidemic accounts describe the same disease later identified as bubonic plague. The Plague of Athens, described in vivid detail by the historian Thucydides beginning in 430 BCE, killed a significant portion of the Athenian population and is thought by some historians to have altered the trajectory of the Peloponnesian War, though the precise illness responsible remains disputed among modern historians of medicine.
The first event widely accepted as a major plague pandemic is the Plague of Justinian, which erupted in 541 CE during the reign of the Byzantine Emperor Justinian I and recurred in waves across the Mediterranean world for roughly two centuries. The historian Procopius left detailed accounts of the epidemic in Constantinople, describing the enormous mortality and the overwhelming of burial infrastructure in the imperial capital. Some historians argue this pandemic contributed meaningfully to the weakening of the Byzantine Empire and to the disruption of Justinian's ambitions to reunify the former Roman world.
The second and most historically consequential pandemic began in Central Asia in the 1340s and reached Europe by 1347, when infected ships docked in the Sicilian port of Messina. The use of quarantine as a social response to epidemic disease developed substantially during this period. The city of Ragusa (modern Dubrovnik) established a formal thirty-day isolation period for arriving ships and travelers in 1377, later extended to forty days — the quarantino from which the English word quarantine derives — representing one of the earliest institutionalized public health measures in European history.
A third pandemic originated in Yunnan province in China in the mid-nineteenth century and spread globally through maritime trade routes in the 1890s. It was during this pandemic that the crucial scientific discoveries were finally made. In 1894, working simultaneously in Hong Kong during an active outbreak, Swiss-French bacteriologist Alexandre Yersin and Japanese bacteriologist Kitasato Shibasaburō both isolated the bacterium responsible. Yersin's identification was ultimately confirmed as correct, and the bacterium was later named Yersinia pestis in his honor.
The role of the rat flea as the vector transmitting the bacterium from infected rodents to humans was demonstrated by French researcher Paul-Louis Simond in 1898, completing a chain of discovery that finally explained why plague had so persistently haunted human settlements near certain animal populations across thousands of years of recorded history.
