Pleurisy occupies a prominent place in the oldest surviving Western medical literature. Hippocrates, writing in the 4th and 5th centuries BCE, devoted specific attention to pleurisy in his Aphorisms and other texts, describing the sharp pain associated with breathing and the fever that accompanied it. He considered pleurisy a serious and frequently fatal condition and made careful observations about which patients were likely to survive based on the course of the illness over its first several days. In the ancient Greek understanding, pleurisy resulted from an excess or morbid accumulation of humors in the pleural space — the area between the lung and the chest wall. Galen elaborated on Hippocratic descriptions and debated the precise humoral mechanism involved, distinguishing between what he called 'true' and 'false' pleurisy based on the location of the pain and the character of the fever. This distinction would persist in various forms through medieval medical writing.

Pleurisy was a condition that appeared repeatedly in the annals of historical deaths. The death of Alexander the Great in 323 BCE has been attributed by some historians and medical scholars to pleurisy, though competing hypotheses exist. The condition was considered sufficiently distinct and significant that Roman physicians treated it as a major medical emergency, and it appears in the writings of Celsus, Caelius Aurelianus, and other Roman medical authors, each offering their own refinements on the Greek descriptions and the recommended treatments of their era.

Medieval Islamic and European physicians inherited the classical framework for understanding pleurisy. Ibn Sina devoted considerable attention to it in the Canon of Medicine, distinguishing between different forms of the condition and offering elaborate explanations based on humoral theory. European scholastic medicine of the 12th through 15th centuries preserved and extended these discussions, with pleurisy remaining one of the recognized acute diseases that physicians were expected to diagnose and treat.

The Renaissance and early modern period brought renewed anatomical investigation that gradually changed the understanding of pleurisy. As physicians began to conduct autopsies more systematically, they could observe directly that the pleura — the membrane lining the chest cavity — showed signs of inflammation and sometimes contained fluid or fibrous deposits in patients who had died with the condition. Giovanni Battista Morgagni's 18th-century pathological work contributed to grounding pleurisy in observable anatomical changes rather than purely humoral theory.

The invention of the stethoscope by René Laënnec in 1816 was particularly significant for pleurisy, as Laënnec identified and described the characteristic sounds produced by inflamed pleural surfaces rubbing against each other during breathing — a sound known as a pleural rub. This gave physicians a way to detect the condition during life through direct physical examination in a manner that had not previously been possible. Over the 19th century, as bacteriology advanced, it became increasingly clear that pleurisy was frequently caused by infectious agents, including the tuberculosis bacterium and the pneumococcus, tying its history directly to the broader story of germ theory's triumph in medicine.
