As families crowded into smoky, narrow urban tenements with minimal sunlight exposure and diets dependent on cheap, nutritionally limited foods, rickets became epidemic across the working-class populations of Britain, Northern Europe, and eventually North America. Reformers and social observers of the 19th century frequently cited the bowed legs and skeletal deformities visible in children of industrial slums as visible evidence of the human cost of rapid industrialization.

The scientific unraveling of rickets accelerated in the early 20th century. Around 1918 and 1919, Edward Mellanby conducted a series of now-famous experiments with dogs in which he demonstrated that the disease had a clear dietary component, identifying that certain fats in food appeared to prevent it. Shortly afterward, Elmer McCollum, building on Mellanby's work, identified the protective factor as a substance distinct from the already-known Vitamin A, naming it Vitamin D in 1922. Concurrently, researchers including Kurt Huldschinsky in Berlin were demonstrating through clinical observation that exposure to ultraviolet light had a dramatic corrective effect on affected children, with Huldschinsky's 1919 experiments under mercury quartz lamps providing compelling photographic evidence.

The convergence of these two lines of inquiry — dietary fat-soluble substances and ultraviolet radiation — eventually led to the understanding that the human body could produce a necessary compound through skin exposure to sunlight, a remarkable biological discovery that fundamentally changed how physicians and public health authorities thought about the relationship between environment and disease. By the 1930s, the fortification of certain common foods with Vitamin D had begun in several industrialized nations as a public health measure, and rickets, which had been among the most widespread childhood conditions of the urban industrial world, began a historic decline. The history of rickets stands as one of medicine's most instructive examples of how a disease that seemed mysterious for centuries could yield to coordinated scientific investigation across multiple disciplines.
