Historical Narrative

Written accounts describing what almost certainly represents scurvy appear in some of the earliest surviving medical texts. The ancient Egyptians documented a condition with similar characteristics around 1550 BCE in the Ebers Papyrus, and Hippocratic writings from ancient Greece reference a disease involving bleeding gums and weakness that scholars have retrospectively identified as consistent with scurvy. In the medieval period, Crusaders and populations enduring long winters or sieges suffered what chroniclers described vividly but without scientific explanation.

Scurvy became truly notorious during the Age of Exploration, beginning in earnest in the 15th century. As Portuguese, Spanish, Dutch, and English vessels embarked on long ocean voyages, the disease became a reliable and dreaded companion. Vasco da Gama's voyage around the Cape of Good Hope in 1497–1499 was reportedly marked by severe scurvy among his crew. Estimates of total deaths attributable to scurvy during the age of sail range into the millions, making it arguably the most lethal nutritional deficiency in the history of seafaring.

The tragedy was compounded by the fact that practical remedies had been observed repeatedly throughout the 16th and 17th centuries. Jacques Cartier, exploring the St. Lawrence River in the 1530s, recorded that Indigenous people showed his ailing crew how to prepare a tea from the bark and needles of the white cedar tree, with reportedly dramatic results. Dutch sailors noted that citrus fruits appeared to ward off the disease. Yet these observations failed to translate into consistent naval policy for generations, partly because of the difficulty of preserving citrus on long voyages and partly because the theoretical frameworks of the era — focused on 'humors,' miasmas, and general debility — did not accommodate the idea that a single missing dietary element could cause catastrophic illness.

The pivotal figure in scurvy's scientific history is the Scottish naval surgeon James Lind, who in 1747 conducted what is now frequently cited as one of the earliest controlled clinical trials in the history of medicine. Aboard HMS Salisbury, Lind systematically tested six different proposed remedies on twelve sailors, ultimately observing that those receiving citrus fruits showed the most dramatic improvement. He published his 'Treatise on the Scurvy' in 1753. However, even Lind himself was not fully convinced of the simple dietary explanation, and it took several more decades before the British Admiralty formally adopted citrus juice as standard naval ration, a policy credited largely to Sir Gilbert Blane and formally implemented in 1795. The immediate subsequent decline in scurvy cases in the Royal Navy was historically striking.

The chemical explanation for scurvy had to wait until the 20th century. Albert Szent-Györgyi isolated a compound from adrenal glands and plant tissues in the late 1920s that was eventually identified as ascorbic acid, named Vitamin C in 1932 through the collaborative work of several researchers including Charles Glen King. Szent-Györgyi received the Nobel Prize in Physiology or Medicine in 1937. Scurvy thus became one of the first diseases to be definitively explained at a biochemical level, closing a historical loop that stretched from Egyptian papyri to the modern laboratory.
