Hippocrates is credited with some of the earliest written observations of what would later be recognized as stroke, using the Greek term 'apoplexy' — derived from a word meaning 'struck down with violence' — to describe the sudden loss of sensation and movement that could befall a person without apparent external injury. The Hippocratic corpus, compiled roughly between the fifth and fourth centuries BCE, included observations noting that apoplexy was more common in certain age groups and body types, though explanations remained rooted in humoral theory.

Galen continued the tradition of writing about apoplexy in the second century CE, theorizing that a surplus of cold, thick phlegm flooded the brain's ventricles and interrupted the flow of 'animal spirits' — a mysterious substance believed to convey sensation and motion through the nerves. This pneumatic theory, blending Galenic anatomy with earlier Greek philosophy, shaped how European physicians understood stroke-like events well into the early modern period.

Throughout the medieval period, apoplexy was commonly interpreted as an act of divine will, particularly in European and Middle Eastern medical traditions. Islamic scholars such as Avicenna (Ibn Sina), writing in the eleventh century, preserved and expanded Galenic thought while adding their own clinical observations. Avicenna's 'Canon of Medicine' discussed apoplexy at length and was used as a reference text in European universities for centuries after his death.

The anatomical revolution of the sixteenth and seventeenth centuries brought the first serious challenges to the pneumatic model of stroke. Johann Jakob Wepfer, a Swiss physician working in the seventeenth century, is regarded as a foundational figure in the history of stroke research. Through careful post-mortem examinations, Wepfer demonstrated in his 1658 work 'Observationes Anatomicae' that apoplexy was associated with bleeding into the brain or with obstruction of the cerebral blood vessels — a vascular explanation that broke sharply from centuries of humoral and pneumatic theory.

Wepfer's findings did not immediately displace older ideas, and debate continued for generations over the relative roles of hemorrhage and obstruction. The eighteenth century saw increasing interest in correlating observable changes in the brain at autopsy with the kinds of events that had occurred during life, a method pioneered more broadly by Morgagni and applied to cerebral cases by subsequent anatomists.

The word 'stroke,' as an English-language term for apoplexy, came into common medical usage during the seventeenth century, reflecting the popular belief that such events represented a sudden blow from an unseen force. Throughout the eighteenth and nineteenth centuries, debates within medical communities increasingly centered on whether strokes resulted primarily from bleeding within the brain, from blockage of cerebral arteries, or from some combination of the two. Physicians including John Cheyne and others contributed detailed case records that helped build a cumulative understanding of how different forms of cerebrovascular event could produce different outcomes, though the tools to distinguish them definitively during life would not arrive until well into the twentieth century.
