Historical Narrative
Among the earliest documented references to thyroid conditions are ancient Chinese medical writings attributed to the era of Emperor Shennong around 2700 BCE, which described visible neck swellings and proposed seaweed and burned sponge as remedies — an empirical solution that, entirely by accident, introduced iodine to patients at a time when no one understood why it helped.
The ancient Romans and Greeks recognized goiter as a geographically patterned condition, noting its prevalence in mountainous inland regions far from the sea. Julius Caesar reportedly observed goiter among Alpine populations during his campaigns, and Roman writers speculated about connections to water quality and mountain air. These geographic observations were astute, even if the underlying reason — iodine-poor soil and water in mountain regions — remained unknown for almost two thousand more years.
The thyroid gland itself received its formal anatomical name from English physician Thomas Wharton in 1656, who named it after the Greek word for shield due to its shape. William Withey Gull in 1873 described the adult form of severe thyroid underactivity, noting the characteristic skin changes and mental slowing in his patients.
Swiss surgeon Theodore Kocher performed extensive thyroid surgery during the latter decades of the nineteenth century and observed that complete removal of the gland produced a state similar to what Gull had described. His meticulous record-keeping over thousands of surgical cases earned him the Nobel Prize in Physiology or Medicine in 1909, the first awarded for surgical work.
The identification of the thyroid's dependence on iodine became concrete through the work of David Marine in the early twentieth century, who demonstrated experimentally that iodine deficiency caused goiter and that supplementation could prevent it. His advocacy contributed to the introduction of iodized salt programs that would reshape the geography of thyroid disease across populations worldwide throughout the twentieth century.
