Historical Narrative

The earliest well-documented epidemic attributed to yellow fever occurred in the Yucatán Peninsula in 1648, recorded by Spanish colonial authorities who described a devastating illness they called 'xekik,' meaning 'blood vomit,' among the Maya population. Some historians have argued for even earlier outbreaks in West Africa, suggesting the disease traveled to the New World aboard slave ships during the sixteenth century, though definitive pre-1648 records remain a subject of scholarly debate.

Throughout the seventeenth and eighteenth centuries, yellow fever terrorized port cities across the Atlantic world. Philadelphia's 1793 epidemic killed roughly one in ten of the city's residents and sent President George Washington and most of the federal government fleeing to the countryside. The city's physicians, led by the prominent Dr. Benjamin Rush, were deeply divided over causation. Rush championed a miasma theory, believing stagnant air rising from rotting cargo on the Delaware River docks was responsible. His insistence on aggressive bloodletting and purging as treatment was later criticized by contemporaries such as Scottish physician William Currie, who favored a contagionist explanation and more conservative interventions.

Napoleon Bonaparte's imperial ambitions in the Western Hemisphere were substantially undone by yellow fever. When French forces arrived in Saint-Domingue in 1802 to suppress the Haitian Revolution, the disease killed an estimated forty thousand soldiers, including commander Charles Leclerc. This catastrophic loss contributed to Napoleon's decision to abandon French ambitions in North America and sell the Louisiana Territory to the United States in 1803.

The nineteenth century brought systematic but still largely incorrect efforts to understand the disease. The Cuban physician Carlos Finlay proposed in 1881 that a particular species of mosquito served as the transmitting agent, a hypothesis that was largely dismissed by the international medical community at the time. It was not until 1900 that a United States Army Yellow Fever Commission, led by Major Walter Reed and including colleagues Jesse Lazear, James Carroll, and Aristides Agramonte, conducted controlled experiments in Cuba that decisively confirmed Finlay's mosquito hypothesis. Lazear died from the disease during the research, and Carroll suffered a serious infection while serving as a deliberate experimental subject.

Armed with this knowledge, engineer William Crawford Gorgas organized mosquito-control campaigns in Havana and later in the Panama Canal Zone, where yellow fever had previously made large-scale construction work nearly impossible. The success of these campaigns between 1905 and 1913 demonstrated for the first time that vector-control could arrest the spread of a major epidemic disease, a principle that would reshape public health practice worldwide.

Max Theiler of the Rockefeller Foundation developed an effective vaccine in the 1930s, work for which he received the Nobel Prize in Physiology or Medicine in 1951. His development drew on decades of earlier laboratory groundwork, including the isolation of the yellow fever virus in 1927 by Adrian Stokes, who himself died from the disease in the course of his research. The history of yellow fever is thus inseparable from a long line of scientists who risked and sometimes lost their lives to understand it.
