Historical Narrative

Ancient Egyptian, Greek, and Roman medical texts preserved accounts of individuals who suffered severe or unusual reactions to substances that caused no harm in others, though ancient physicians lacked any theoretical apparatus to explain such selectivity. Hippocratic writers attributed what appeared to be exceptional bodily responses to unique constitutional temperaments, arguing that individuals possessed different humoral compositions that made them more or less susceptible to various external influences. This framework of individual constitution persisted through medieval Islamic medicine, where scholars such as Ibn Sina incorporated similar ideas into comprehensive medical encyclopedias that guided European and Middle Eastern practice for centuries.

The transformation of hypersensitivity from a folk observation into a subject of rigorous scientific inquiry began in the late nineteenth century, when bacteriology and experimental immunology were reshaping medicine's foundations. The pioneering work of scientists investigating immunity focused primarily on the body's capacity to resist disease, but careful observers soon noticed that the same immunological processes could produce harmful reactions under certain conditions. The French physiologist Charles Richet and his colleague Paul Portier conducted crucial experiments in the early twentieth century that formalized this darker face of immunity.

Richet and Portier, working initially with sea anemone toxins aboard Prince Albert I of Monaco's research yacht in 1901, discovered that dogs previously exposed to small doses of a toxin reacted with extreme and sometimes fatal violence upon subsequent exposure, rather than showing the increased resistance that prevailing immunological theory would have predicted. Richet coined the term anaphylaxis to describe this phenomenon, deliberately contrasting it with prophylaxis to highlight that the immunological process could harm rather than protect. His discovery earned him the Nobel Prize in Physiology or Medicine in 1913 and permanently altered the conceptual landscape of immunology.

Simultaneously, the Austrian pediatrician Clemens von Pirquet was developing a broader theoretical framework that could encompass the full spectrum of altered immunological reactivity. Von Pirquet introduced the term allergy in 1906, deriving it from Greek roots meaning altered reactivity, to describe both the beneficial and harmful changes in bodily responses that followed exposure to foreign substances. His framework was deliberately inclusive, recognizing that immunity and hypersensitivity were expressions of the same underlying biological processes rather than fundamentally different phenomena.

The British physician Geoffrey Langdon Keynes and later researchers in the mid-twentieth century contributed to mapping the different forms that hypersensitivity reactions could take. The most enduring classification system emerged from the collaborative work of Philip Gell and Robin Coombs, two British immunologists who in 1963 published a schema dividing hypersensitivity reactions into four distinct types based on their underlying mechanisms. The Gell and Coombs classification organized decades of disparate clinical and experimental observations into a coherent framework and became a cornerstone of immunological education throughout the latter twentieth century.

Earlier in that century, the discovery of immunoglobulin E by Japanese researchers Kimishige and Teruko Ishizaka in the 1960s illuminated the molecular basis of one major category of hypersensitivity reactions, providing a biochemical anchor for phenomena that clinicians had observed and debated since Richet's foundational experiments. This discovery connected the visible drama of anaphylactic and allergic reactions to specific molecular actors, closing a chapter of mechanistic uncertainty that had persisted for more than half a century.
