Historical Narrative

The pituitary gland was known to ancient anatomists, who observed the small structure at the base of the brain and attempted to assign it a purpose consistent with their prevailing theories of the body. Galen, the second-century Roman physician whose writings dominated European medical thought for over a thousand years, proposed that the pituitary served as a conduit through which waste materials, specifically a substance he called phlegm or pituita, drained from the brain into the nasal cavity. This interpretation, embedded deeply in Galenic anatomy, meant that the gland was understood as a passive drainage organ rather than a gland of secretion, and the idea persisted with remarkable durability through the medieval period and into the Renaissance.

Andreas Vesalius, the Flemish anatomist whose 1543 work De Humani Corporis Fabrica revolutionized the study of human anatomy, described the pituitary with greater precision than his predecessors but remained within a broadly Galenic interpretive framework. Renaissance anatomists continued to depict the gland primarily in the context of its supposed drainage function, and it was not subjected to sustained experimental investigation for some time after Vesalius's era.

Significant reinterpretation began in the seventeenth century when Marcello Malpighi and other microscopists examined glandular structures throughout the body and began reconsidering the role of small glands as secretory organs rather than mere conduits. However, the specific functions of the pituitary remained obscure for another two centuries. It was not until the latter half of the nineteenth century that clinical observers began connecting abnormalities of the pituitary with distinct patterns of disease. The French neurologist Pierre Marie described acromegaly in 1886 and linked it to pituitary tumors, a landmark observation that helped establish the gland's role in regulating growth and provided the first clear clinical evidence of pituitary pathology producing systemic effects.

The specific syndrome later understood as hypopituitarism began to take clearer shape through the work of the German physician Morris Simmonds, who in 1914 described a condition of severe wasting and glandular atrophy in a patient whose pituitary was found to be largely destroyed at autopsy. Simmonds connected the destruction of pituitary tissue to the broad constellation of physical changes the patient had exhibited, and the syndrome was subsequently known as Simmonds' disease for several decades. His observations prompted other clinicians to look more systematically at patients presenting with similar patterns of physical decline and to examine the pituitary post-mortem.

Harold Leeming Sheehan, a British pathologist working in the mid-twentieth century, made a particularly important contribution by studying women who had suffered severe hemorrhage during childbirth and subsequently developed progressive failure of multiple endocrine glands. Sheehan published his findings in 1937, describing how catastrophic blood loss could cause infarction of the pituitary, and the resulting syndrome of pituitary insufficiency following obstetric hemorrhage came to bear his name. His work was conducted through careful clinicopathological correlation, matching the histories of affected women with post-mortem findings.

Parallel advances in experimental endocrinology, driven by researchers such as Herbert Evans and Philip Smith in the United States during the 1920s and 1930s, established through animal experiments that removal of the pituitary produced widespread failure of other endocrine glands, confirming the gland's role as a master regulator and providing an experimental foundation for understanding the consequences of its insufficiency in human patients.
