The conditions now understood to result from thyroid hormone deficiency left visible marks on populations throughout recorded history, and ancient texts from multiple civilizations described individuals bearing the outward signs of severe thyroid insufficiency. Ancient Chinese medical writings and Roman authors referenced populations in alpine and inland regions where goiter was endemic, and skeletal evidence from medieval European populations has revealed features associated with congenital thyroid deficiency. However, ancient and medieval healers possessed no framework in which the thyroid gland could be understood as a producer of any essential internal substance, and the gland itself was not consistently recognized as a discrete anatomical structure.

Ancient anatomists gave various accounts of the structures in the neck, but the thyroid gland as a named entity did not receive clear recognition until the Renaissance. Thomas Wharton, the seventeenth-century English anatomist, provided one of the first detailed descriptions of the thyroid gland as a distinct organ in his 1656 work Adenographia, though he believed its primary purpose was to warm the larynx and lubricate the trachea. The possibility that the thyroid produced any substance essential to bodily function did not enter mainstream medical thinking for another two centuries.

Medieval and early modern physicians encountered the dramatic swelling of the thyroid known as goiter with particular frequency in mountainous regions, where iodine-deficient water sources were later found to be prevalent. Early remedies drawn from empirical observation included the prescription of seaweed, burnt sponge, and other marine substances, treatments that physicians prescribed without any understanding of why they sometimes appeared effective. The concept that these remedies contained an element, later identified as iodine, which the thyroid specifically required, lay entirely beyond the reach of pre-nineteenth-century medical thought.

The description of cretinism, the severe developmental condition resulting from thyroid deficiency in infancy, attracted considerable medical and social attention in eighteenth-century Europe, particularly in Switzerland and other alpine regions where the condition appeared with disturbing frequency. Physicians and natural historians of the Enlightenment documented cretinism carefully, debating whether it arose from environmental factors, hereditary constitution, or the consumption of contaminated water.

The decisive turning point in understanding hypothyroidism came in the 1870s and 1880s, when a series of clinical and experimental investigations established the essential role of the thyroid gland. In 1873, William Withey Gull of Guy's Hospital published a landmark description of a cretinoid state developing in adult women, describing patients whose skin had taken on a peculiar doughy quality and whose mental and physical capacities had progressively declined. The clinical syndrome Gull described was later given the name myxedema by William Ord, who in 1878 performed post-mortem examinations revealing a mucoid infiltration of the skin.

Experimental confirmation of the thyroid's essential role came from surgical studies in the 1880s. Moritz Schiff had conducted thyroid removal experiments on animals decades earlier, but it was Victor Horsley and others working in this period who built the experimental and clinical evidence connecting thyroid removal to the development of myxedema-like states. The Clinical Society of London published an influential report on myxedema in 1888, synthesizing the accumulated evidence and firmly establishing thyroid deficiency as the condition's cause. By the 1890s, physicians were experimenting with the administration of thyroid extracts to patients with myxedema, marking the beginning of a new era in the condition's management.
