Among the oldest medical texts in human history, the Ebers Papyrus from ancient Egypt, dating to approximately 1550 BCE, described conditions of pallor and weakness that later historians came to associate with iron deficiency anemia. Egyptian healers prescribed preparations containing ochre and other iron-rich earths, though their reasoning was rooted in ritual and sympathetic correspondence rather than any understanding of nutrition. In ancient China, texts from the Han dynasty similarly recorded pale, listless patients and recommended consuming certain animal livers and dark leafy preparations, again without any mechanistic understanding of why such remedies might work.

Ancient Greek physicians, working within the framework of humoral medicine, interpreted the pallor and weakness characteristic of iron deficiency as an excess of phlegm or a deficit of blood. Hippocratic writings from the fifth and fourth centuries BCE described a condition called chlorosis in young women, marked by a greenish pallor, which historians later debated as a possible description of iron deficiency anemia. The name chlorosis, derived from the Greek word for green, persisted in European medical literature for over two thousand years.

During the medieval period, European physicians continued to treat chlorosis and related wasting conditions with herbal tonics, bloodletting, and dietary regimens prescribed according to humoral principles. Ironically, bloodletting — the removal of blood — was sometimes applied even to patients whose fundamental problem was insufficient blood quality, a reflection of how thoroughly humoral theory dominated clinical reasoning. Arabic physicians such as Ibn Sina, known in the West as Avicenna, wrote extensively on blood disorders in the eleventh century and recommended iron-containing mineral waters and liver preparations, edging closer to an empirical association between iron and vitality without grasping the underlying mechanism.

The Renaissance brought renewed interest in chemical medicine, and Paracelsus in the sixteenth century advocated for iron compounds as therapeutic agents, framing their power in alchemical rather than nutritional terms. He believed iron carried a martial virtue that could reinforce weakened vital spirits. Despite the mystical framing, iron-based preparations became increasingly common in European apothecaries.

A pivotal conceptual shift came in the seventeenth century when the English physician Thomas Sydenham championed iron-rich preparations, particularly iron filings dissolved in wine, as a remedy for chlorosis. Sydenham's empirical approach helped cement the association between iron and the treatment of pale, fatigued patients, and his recommendations were influential throughout Europe for generations.

The nineteenth century brought the tools of analytical chemistry to bear on the question. French chemist Antoine François de Fourcroy and later researchers demonstrated that iron was a normal constituent of blood. In the 1820s and 1830s, chemists including Jöns Jacob Berzelius and later researchers more precisely characterized hemoglobin and its iron content. By the latter half of the nineteenth century, physicians began connecting dietary iron insufficiency to the clinical picture of anemia in a more systematic way.

The early twentieth century work of George Whipple, George Minot, and William Murphy, though primarily focused on pernicious anemia, stimulated broader scientific interest in nutritional anemias. Researchers in subsequent decades mapped the pathways of iron absorption, transport, and utilization, finally providing a biochemical foundation for what healers had observed and fumbled toward treating for three millennia.
