The Sushruta Samhita, a foundational Sanskrit medical text, contained descriptions of a condition called kushtha that many historians have interpreted as leprosy, placing its recognition in Indian medicine as early as 600 BCE or possibly earlier. Egyptian papyri from roughly the same era referenced skin conditions that some scholars identified as consistent with leprous changes, though ancient terminology was imprecise enough to encompass multiple skin diseases under a single name. In the Hebrew Bible, the term tzara'at was used to describe skin afflictions that required priestly examination and could result in isolation from the community, and for centuries Western interpreters equated this with leprosy, though modern scholarship has cast doubt on that identification. Greek physicians including Hippocrates described a condition they called elephantiasis graecorum, noting the thickening and disfigurement of the skin, and this term persisted in medical literature for many centuries as a name for what would eventually be recognized as leprosy. Galen and later Roman medical writers incorporated similar descriptions into their compilations, generally following humoral theory in attributing the disease to an excess of black bile. Medieval European understanding was heavily shaped by religious interpretation, and the Church played a central institutional role in managing those diagnosed with leprosy. Elaborate ecclesiastical ceremonies developed during the medieval period in which individuals diagnosed with the disease were formally declared civilly dead, symbolically separated from the community, and required to carry bells or clappers to warn of their approach. Leprosaria, also called lazar houses or leper hospitals, proliferated across medieval Europe from roughly the eleventh through the fourteenth centuries, housing those excluded from ordinary society. The decline of these institutions after the fourteenth century has been attributed by historians to various factors including changes in the disease's prevalence, competition from plague, and shifts in social organization. A transformative moment in the scientific understanding of leprosy came in 1873 when Norwegian physician Gerhard Armauer Hansen identified a rod-shaped bacterium in the tissues of leprosy patients, which he correctly proposed as the causative agent. This organism, eventually named Mycobacterium leprae in recognition of his work, became one of the first bacteria definitively linked to a chronic human disease, predating Koch's postulates and placing Hansen among the pioneers of the germ theory era. Hansen's discovery faced initial resistance, particularly from his mentor Daniel Cornelius Danielssen, who had spent decades studying the disease and believed it to be hereditary rather than infectious. Danielssen and Carl Wilhelm Boeck had produced a landmark descriptive work on leprosy in 1847 that represented the most thorough clinical account of the disease to that point, but their hereditary theory was ultimately displaced by Hansen's infectious etiology. The development of promin, a sulfone drug, in the 1940s by researchers at the United States Public Health Service facility at Carville, Louisiana, marked the first effective pharmaceutical intervention against the disease, closing a chapter of millennia during which no curative agent existed.
