The historical record of leptospirosis as a recognized clinical entity begins most clearly in the late nineteenth century, though retrospective analyses have suggested that earlier accounts of febrile jaundice among workers in mines, sewers, and rice fields may have described the same condition under various names. European physicians working in tropical colonial territories during the eighteenth and nineteenth centuries documented severe jaundice and fever among agricultural laborers and soldiers, producing accounts that differed from known forms of malaria and yellow fever without offering an alternative explanation.

Adolf Weil, a German physician practicing in Heidelberg, published in 1886 a systematic clinical description of a severe febrile illness characterized by jaundice, kidney involvement, and hemorrhagic features in a group of patients. His careful delineation of this syndrome as a distinct entity earned the severe form of leptospirosis the eponym Weil's disease, which persisted in medical literature for over a century. Weil himself did not identify a causative organism, and his work focused on defining the clinical picture and distinguishing it from other forms of jaundice then being intensively studied.

The bacterial cause of the disease remained unknown for nearly three more decades after Weil's description. During this period, physicians in Japan, Europe, and the Americas continued to document outbreaks among workers in flooded fields, abattoir workers, sewer cleaners, miners, and soldiers operating in waterlogged trenches. The First World War dramatically increased medical awareness of the condition, as troops in the flooded trenches of the Western Front experienced severe outbreaks of jaundice that military physicians struggled to explain and manage.

The bacteriological breakthrough came in 1914 and 1915, when Japanese researchers made the decisive discovery. Ryukichi Inada and Yutaro Ido, working in Japan and subsequently in China, successfully identified and cultured a spiral-shaped bacterium from the blood and tissues of affected patients and experimentally infected guinea pigs. They named the organism Spirochaeta icterohaemorrhagiae, and their work established for the first time that a specific microorganism caused Weil's disease. This discovery was one of the landmark achievements of early twentieth century tropical medicine.

Almost simultaneously, German researchers including Hubert Uhlenhuth and Wilhelm Fromme working in the field during the First World War independently described a similar organism, leading to a period of parallel and overlapping scientific publications. The subsequent decades brought taxonomic revision of the causative organism, eventually classified within the genus Leptospira, and researchers began recognizing that numerous related bacterial variants caused overlapping syndromes across different geographic regions and animal reservoirs.

The interwar period saw the establishment of leptospirosis as a significant occupational and tropical disease concern across European colonial medical services. Researchers in the Dutch East Indies, British India, and Australia contributed case series and epidemiological observations connecting the disease to contact with water contaminated by the urine of infected rodents and domestic animals. The role of rats as a primary reservoir had been suggested by early bacteriologists and was progressively confirmed through experimental animal studies, reshaping how public health authorities of that era conceptualized disease prevention in occupational and military contexts.
