René Descartes's seventeenth-century philosophical dualism, which sharply separated mind from body and located consciousness in something other than physical motion, inadvertently provided one such framework, though Descartes himself never applied it to clinical neurology. The emergence of systematic neuroanatomy through the work of Thomas Willis in the seventeenth century and then through the detailed clinicopathological studies of the eighteenth and nineteenth centuries gradually established that damage to different brain regions produced different deficits.

Alexandre Dumas's 1844 novel The Count of Monte Cristo contained what many later commentators recognized as a prescient literary portrait of locked-in syndrome in the character of Monsieur Noirtier de Villefort, a man left entirely paralyzed by a stroke who communicated only through eye movements and who was understood by his family to retain full intelligence. Whether Dumas based this character on an observed case or on medical literature of his era was not established, but the literary depiction predated any formal medical description of the syndrome by well over a century.

Formal neuropathological understanding of the brainstem lesions capable of producing this clinical picture advanced through the nineteenth century as physicians including Jean-Martin Charcot at the Salpêtrière hospital in Paris conducted meticulous clinicopathological correlation studies. Charcot and his collaborators identified specific brainstem pathways responsible for voluntary movement and described the devastating consequences of their disruption, though they did not assemble these observations into a defined syndrome of preserved consciousness with total motor paralysis.

The term locked-in syndrome was formally coined by Fred Plum and Jerome Posner in their 1966 textbook The Diagnosis of Stupor and Coma, a landmark work that brought systematic rigor to the clinical assessment of consciousness disorders. Plum and Posner precisely defined the syndrome as the combination of quadriplegia, anarthria, and preserved vertical eye movements with intact consciousness, arising from ventral pontine lesions, and distinguished it clearly from vegetative states and coma. This conceptual separation was of profound humanitarian significance, since it implied that patients who had previously been treated as unconscious or dying might in fact be fully aware.

In subsequent decades, communication researchers and neurologists including the French journalist Jean-Dominique Bauby, who dictated his memoir The Diving Bell and the Butterfly entirely through eye blinks following a brainstem stroke in 1995, brought public attention to the preserved inner life of affected individuals. Bauby's account, published in 1997 shortly before his death, became perhaps the most widely known testimony to the condition's history and to the long centuries during which such experiences had gone unrecognized.
