Lymphoma's history as a recognized medical category began in the early nineteenth century when physicians first began distinguishing diseases of the lymphatic system from other conditions that caused enlarged glands and organ swelling. For much of history, swellings of the lymph nodes had been attributed to tuberculosis, syphilis, or generalized constitutional weakness. The gradual development of pathological anatomy and microscopy allowed physicians to separate lymphoma from these other conditions and eventually to recognize it as a family of distinct malignant diseases.
Historical Narrative
Before lymphoma was recognized as a disease entity, swollen lymph nodes were almost universally interpreted by healers throughout antiquity and the medieval period as signs of infection or humoral imbalance. Hippocratic physicians spoke of enlarged glands as consequences of corrupt phlegm accumulating in the body, and the remedies prescribed reflected this understanding — purging, bloodletting, and the application of caustic substances to soften the swellings were common historical interventions. Medieval Arabic physicians, including Ibn Sina, whose encyclopedic Canon of Medicine shaped European medical thinking for centuries, described glandular swellings in detail but grouped them together with tubercular and inflammatory conditions under broad humoral categories.
The first definitive step toward recognizing lymphoma as a distinct pathological entity came with the work of Thomas Hodgkin, an English physician at Guy's Hospital in London. In 1832, Hodgkin published a landmark paper titled 'On Some Morbid Appearances of the Absorbent Glands and Spleen,' in which he described the postmortem findings in seven patients who had died with dramatically enlarged lymph nodes and spleens. Hodgkin carefully distinguished these cases from tuberculosis and inflammation, arguing that the changes represented a primary disease of the lymphatic system. His observations were based entirely on gross anatomical examination, as the microscope had not yet become a standard tool of pathological investigation.
For nearly three decades, Hodgkin's paper attracted little attention. It was Samuel Wilks, another physician at Guy's Hospital, who rediscovered and championed Hodgkin's original observations in the 1860s. Wilks added additional cases, formally named the condition 'Hodgkin's disease' in honor of his predecessor, and helped establish it in the medical literature as a recognized clinical and pathological entity.
The development of histological techniques using the microscope allowed late nineteenth-century pathologists to examine lymph node tissue at the cellular level. Carl Sternberg in 1898 and Dorothy Reed in 1902 independently described the distinctive giant cells that came to bear both their names — the Reed-Sternberg cells — which became the defining microscopic hallmark that pathologists used to identify the condition. Reed's detailed histological work was conducted at Johns Hopkins and represented one of the significant early contributions of a woman to American pathological research.
Meanwhile, pathologists in the late nineteenth and early twentieth centuries worked to describe the broader family of lymphatic malignancies that did not share the features of Hodgkin's disease. The German pathologist Rudolf Virchow had earlier described 'lymphosarcoma,' and subsequent researchers gradually built a taxonomy of lymphatic tumors based on cellular appearance and tissue architecture.
Early treatment attempts for lymphoma were largely ineffective. Surgeons removed accessible nodes, and from the late nineteenth century, physicians experimented with the newly discovered X-rays as a therapeutic tool following Wilhelm Röntgen's 1895 discovery of radiation. American physician Nicholas Senn reported attempts to use X-rays against lymphatic tumors as early as 1903. The use of arsenic compounds also had a place in early twentieth-century treatment trials. The modern era of chemotherapy for lymphoma began to take shape only in the mid-twentieth century, growing partly from observations made during World War II about the effects of nitrogen mustard compounds on lymphoid tissue, a discovery that arose from wartime chemical weapons research.
