Aristotle himself remarked upon people who squinted and brought written materials near their eyes, though he did not frame this as a disease so much as a natural variation in human sight. Islamic scholars of the medieval era made substantial contributions to the understanding of optics and the eye. Ibn al-Haytham, the eleventh-century polymath known in the Latin West as Alhazen, composed his landmark Book of Optics, in which he argued that vision resulted from light rays entering the eye rather than emanating from it. His geometrical analysis of refraction laid the groundwork for future understanding of how the eye's shape influenced the focusing of light, though he did not explicitly theorize myopia as a refractive error in the modern sense.

The invention of spectacles in late thirteenth-century Italy marked a pivotal moment in the practical history of myopia. Craftsmen in Pisa and Venice began grinding glass lenses that, when held before the eyes, allowed individuals with poor distant vision to see more clearly. Early spectacle makers operated largely by trial and error, and the lenses they produced for those with distance difficulties were concave in shape. Documents from the thirteenth and fourteenth centuries referenced these instruments, and portraits from the Renaissance period began depicting scholars and merchants wearing rudimentary eyeglasses.

Johannes Kepler, in the early seventeenth century, provided a more rigorous optical explanation of myopia, describing how the elongated eyeball caused parallel light rays to converge at a point in front of the retina rather than upon it. This geometric account replaced the older pneumatic and humoral explanations with a mechanical model grounded in the new science of optics. Kepler's work influenced subsequent generations of natural philosophers and physicians who studied vision.

By the eighteenth and nineteenth centuries, European ophthalmologists began to investigate myopia with increasing systematism. Franciscus Donders, the Dutch ophthalmologist working in the mid-nineteenth century, produced a comprehensive classification of refractive errors, placing myopia within a framework of measurable optical deviation. His work encouraged the development of more precisely ground corrective lenses and helped establish ophthalmology as a rigorous medical discipline. Helmholtz's invention of the ophthalmoscope in 1851 further enabled physicians to examine the interior of the eye and observe the structural characteristics associated with high degrees of myopia.
