For most of Western medical history, malignant tumors were understood primarily through the lens of Galenic humoralism, with cancer attributed to a morbid accumulation of black bile corrupting tissues. This framework persisted with remarkable durability, and internal tumors that did not present as obvious surface masses or accessible swellings were largely encountered only at post-mortem examination. Ancient Egyptian, Greek, and Roman sources contained scattered references to internal growths causing wasting and pain, but the cellular identity of any particular tumor was unknowable before the advent of microscopy.
The emergence of pathological anatomy in early modern Europe began to shift understanding. Andreas Vesalius in the sixteenth century and his successors dissected and catalogued internal organs, occasionally noting unusual growths in the pancreas, intestines, and other abdominal structures. However, the conceptual leap required to identify tumors of hormone-secreting neural-crest-derived cells as a distinct class awaited both the microscope and the germ theory era's emphasis on cellular specificity.
Rudolf Virchow's cellular pathology, established definitively by his 1858 work 'Die Cellularpathologie,' provided the foundational principle that all disease, including cancer, was rooted in cellular abnormality. This transformed pathology and gave investigators the tools to begin discriminating between tumor types based on cellular appearance. It was within this tradition that pathologists in the late nineteenth century began encountering small intestinal and pancreatic tumors with unusual microscopic architecture.
Siegfried Oberndorfer's 1907 description of carcinoid tumors introduced the concept that some of these growths occupied an ambiguous zone between benign and malignant behavior, a distinction that fascinated oncologists and pathologists for decades. Yet even Oberndorfer revised his original relatively benign characterization in later years, acknowledging metastatic spread in some cases.
