Historical Narrative

The earliest systematic attempts to describe disorders of the optic nerve were embedded within ancient Greek medical philosophy, where vision loss unaccompanied by obvious external injury was attributed to humoral imbalance. Galen of Pergamon, writing in the second century, theorized that the optic nerves served as conduits for a vital visual spirit flowing from the brain, and that any obstruction or corruption of this spirit could extinguish sight. Medieval Islamic physicians, particularly Ibn Sina in his eleventh-century Canon of Medicine, elaborated on these ideas, cataloguing forms of blindness he believed arose from weakness or blockage within the hollow optic pathways as Galenic anatomy described them.

For many centuries, physicians had no means of examining the interior of the eye, so conditions affecting the optic nerve were diagnosed almost entirely through observation of the patient's reported experience and the exclusion of more visible causes of blindness such as cataract. The optic nerve itself remained largely inaccessible to clinical investigation, and inflammatory conditions affecting it were grouped loosely with other mysterious visual failures under umbrella terms that varied by region and era.

The invention of the ophthalmoscope by Hermann von Helmholtz in 1851 transformed the study of optic nerve disease entirely. For the first time, physicians could directly observe the optic disc at the back of the eye. Albrecht von Graefe, one of the foremost ophthalmologists of the nineteenth century, used this new instrument to characterize what he called optic neuritis, describing the swollen, inflamed appearance of the disc in patients who had lost vision. Von Graefe's observations in the 1850s and 1860s helped establish optic neuritis as a distinct pathological entity rather than a vague category of unexplained blindness.

Early investigators debated whether the inflammation originated within the nerve itself or spread from surrounding structures such as the meninges or the brain. The concept of retrobulbar neuritis, in which the disc appeared normal to ophthalmoscopic examination despite profound vision loss because the inflammation lay behind the eye, was articulated in the latter decades of the nineteenth century and caused considerable confusion. The aphorism attributed to various Victorian clinicians — that in retrobulbar neuritis the patient sees nothing and the doctor sees nothing either — captured the diagnostic frustration of the era.

The connection between optic neuritis and diseases of the central nervous system began to be appreciated in the late nineteenth and early twentieth centuries. Wilhelm Uhthoff described in 1890 the phenomenon in which vision worsened with physical exertion or heat in patients who had suffered optic nerve inflammation, an observation that bore his name for generations afterward. Neurologists of the early twentieth century increasingly noted that optic neuritis appeared in patients who also developed other neurological signs over time, and this association was progressively linked to what was then being described as disseminated sclerosis. The work of Jean-Martin Charcot in France on the plaques of multiple sclerosis had already laid groundwork for understanding demyelinating disease, and later researchers drew explicit connections between that pathological process and optic nerve inflammation.

By the mid-twentieth century, the demyelinating nature of optic neuritis was widely accepted in neurological and ophthalmological circles, and the condition had shifted from an ophthalmological curiosity to a recognized manifestation of a broader disease process involving the nervous system's myelin sheath.
