For most of recorded history, the trembling and postural changes associated with the condition were attributed to supernatural forces, humoral imbalance, or simple old age rather than to any specific disease of the nervous system. The written record of what later physicians would call Parkinson's disease stretches back several thousand years. Ancient Indian medical texts composed within the Ayurvedic tradition, dating to approximately 1000 BCE, described a condition called Kampavata, characterized by trembling of the limbs, slowness of movement, and an expressionless face. Ayurvedic practitioners treated Kampavata with preparations derived from the seeds of Mucuna pruriens, a tropical legume whose properties would attract renewed scientific attention in the modern era. Ancient Chinese medical literature similarly recorded conditions involving involuntary shaking, and healers prescribed acupuncture and herbal formulas intended to calm what they interpreted as disturbances in the flow of vital energy.
In the Western tradition, the Greek physician Galen of Pergamon, writing in the second century CE, drew a distinction between tremors that occurred at rest and those that occurred during movement, a clinical observation that would prove remarkably durable. Galen attributed resting tremors to an excess of cold, phlegmatic humors weakening the muscles and nerves, and he prescribed warming remedies including wine, heat applications, and tonics. The decisive transformation came with James Parkinson, a London surgeon and political pamphleteer who in 1817 published An Essay on the Shaking Palsy. Drawing on observations of six patients, only three of whom he had formally examined, Parkinson described with considerable precision the gradual onset of tremor, the involuntary festinating gait, and the progressive loss of muscular control that together constituted what he termed paralysis agitans. Parkinson's essay attracted modest attention initially, but French neurologist Jean-Martin Charcot, working at the Salpêtrière hospital in Paris during the latter half of the nineteenth century, championed and substantially extended Parkinson's clinical account. Charcot refined the description of the condition, emphasized the importance of rigidity as a feature distinct from tremor, and proposed that the condition be named after Parkinson in recognition of the priority of his observations. Charcot also experimented with belladonna alkaloids as a treatment, finding that certain preparations reduced salivation and tremor in affected patients.
The twentieth century brought neurochemical and pathological discoveries that fundamentally reshaped the understanding of Parkinson's disease. Friedrich Lewy, a German-American neurologist, identified the intracellular protein inclusions in the early 1910s that would bear his name and become recognized as a hallmark pathological finding. In the 1950s and 1960s, researchers including Arvid Carlsson, Oleh Hornykiewicz, and Herbert Ehringer traced the condition to a deficiency of dopamine in the substantia nigra, a region of the midbrain, establishing the neurochemical basis that would guide much of the subsequent century's research.
